This Cardholder Agreement (“Agreement”) sets forth the terms and conditions under which Central Bank of Kansas City (“CBKC” or “Issuer”) has issued the Ticket Restaurant Prepaid Mastercard® to you to access funds made available to you by your Program Sponsor to make qualifying meal purchases at local restaurants, cafes, bakeries, supermarkets and grocery stores, caterers, convenience stores, other specialty markets, and vending machines pursuant to your Benefits Plan. Please read this Agreement carefully and keep it for future reference. By activating the Card or by loading, using, or authorizing the use of the Card, you represent and warrant to us that: (i) you are at least 18 years of age (or 19 if you reside in a state where the age of majority is 19); (ii) you are a U.S. citizen or legal alien residing in one of the 50 states or the District of Columbia; and (iii) you received a copy of this Agreement and agree to be bound by and to comply with its terms, including the Arbitration
