Without Proof of Purchase. Each Claimant who does not submit sufficient Proof of Purchase, or no Proof of Purchase at all, but who submits an attestation of his/her purchase of Product during the Class Period under penalty of xxxxxxx, may claim one dollar ($1.00) per Product purchased during the Class Period, up to five (5) purchases, for a maximum potential recovery of five dollars ($5.00) and a minimum benefit of three dollars ($3.00).
Without Proof of Purchase. Settlement Class Members who do not have valid Proof(s) of Purchase may submit a Claim Form and recover up to of $1.00 per Product up to five Products or $5.00 per Household.
Without Proof of Purchase. Settlement Class Members who do not have Proof of Purchase may recover up to one dollar and zero cents ($1.00) per Product purchased during the Class Period, up to five purchases, for a maximum potential recovery of five dollars and zero cents ($5.00) and a minimum benefit of three dollars and zero cents ($3.00).
Without Proof of Purchase. Settlement Class Members may make a claim for every Product for which they submit a Valid Claim Form. Settlement Class Members may seek reimbursement at fifty cents ($0.50) per package for every Product purchased during the Class Period, with an anticipated maximum recovery of twenty-five dollars ($25.00) per household.
Without Proof of Purchase. Each Claimant who does not submit sufficient Proof of Purchase, or no Proof of Purchase at all, but who submits an attestation of his/her purchase of Godiva Chocolate Products during the Class Period under penalty of perjury, may claim $1.25 per Godiva Chocolate Product purchased during the Class Period, up to a maximum recovery of $15.
Without Proof of Purchase. You can get up to $.80 per Unit purchased up to a maximum of 6 Units per Household for up to a maximum reimbursement of $4.80 per Household.
Without Proof of Purchase. For those Settlement class members who neither return the Settlement Class Products nor provide a valid receipt or a retail rewards submission, nor whose purchases appear in Defendants’ records, but who substantiate their claims through a submission of an Affidavit attesting to their purchase of the Settlement Class Products under penalty of perjury, together with additional information requested by the Settlement Administrator on the Claim Form, a total of $5.00.
Without Proof of Purchase. Subject to the terms and conditions of this Agreement and approval of the Claims Administrator, Settlement Class Members who, in accordance with the terms of this Agreement, submit to the Claims Administrator a timely, valid, and approved Claim Form for up to and including ten (10) Products purchased per household during the Settlement Class Period, shall be entitled, as of the Effective Date, to a payment in the amount of One and No/100 Dollars ($1.00) per Product purchased, subject to the adjustments set forth in Section IV.B.7. The maximum amount that will be paid to any one Authorized Claimant household without Proof of Purchase will be Ten and No/100 Dollars ($10.00).
Without Proof of Purchase. For class members who submit a Claim Form without a Proof of Purchase that shows a purchase of an Adore Product, Defendants will provide one $50 electronic gift card per such class member for use on Universal Handicraft, Inc.’s website, xxxxx://xxx.xxxxxxxxxxxxxx.xxx/, with a one-year expiration date. The combined total value of all gift cards issued to Authorized Claimants who filed a Claim without Proof of Purchase shall not exceed $100,000 in gift cards.
Without Proof of Purchase. For Settlement Class Members who do not have proof of purchase, Defendants will pay $3.00 per Product purchased during the Class Period up to a limit of 4 bones per household.
