Wellness Services. Covered Services include access to the following wellness related services:
Wellness Services. Wellness services include workshops, trainings, group sessions, and individual coaching sessions to promote optimum well-being. I will draw from my training and experiences to help you put your dreams into achievable goals. Insurance is not accepted for wellness services because I will not be identifying or treating a mental health diagnosis. I honor the time you have devoted to creating a more fulfilling life and will partner with you to develop strategies to help you reach your personal or professional goals.
Wellness Services. FFHC provides certain wellness related services as more fully outlined in Exhibit A (“Wellness Services”) designed to complement the Enhanced Primary Care Services.
Wellness Services. DPMWC provides certain wellness related services as more fully outlined in Exhibit A (“Wellness Services”) designed to complement the Enhanced Primary Care Services.
Wellness Services. A main component of a membership at MHCLT is access to many types of wellness-based activities and practitioners. MHCLT will attempt to provide multiple opportunities per month for You to attend and explore wellness-based activities, e.g. massage, yoga therapy, etc. These activities may occur either at the MHCLT location or at other facilities or locations that are suitable for such activities. These activities will be led and performed only by licensed professionals in their relevant fields of expertise. MHCLT will make every reasonable effort to provide as many wellness-based activities as possible per month, but cannot guarantee the frequency or type of activities that will be available. Excluded Services The following services are not part of the Services in Your membership: ● Specialists. From time to time, You may need to use the care of specialists (e.g. cardiologists, obstetricians, etc.), emergency rooms, urgent care centers, etc. that are outside the scope of this Agreement. Physicians and other staff at MHCLT will coordinate with the specialists you select or to whom You are referred to assist You in obtaining specialty care, and will make an appropriate determination about the scope of primary care services offered by Your Physician on a case-by-case basis. Because no insurance is involved, Your Physician has no incentive to refer You to any particular specialist, which means You and Your Physician can work together to determine the best specialist for You. You understand that fees paid under this Agreement do not include and do not cover specialist’s fees or fees due to any medical professional or service other than a Physician practicing at MHCLT.
Wellness Services. HRH will provide the wellness services set forth on Exhibit B (the "Wellness Services") during the term of this Agreement. Certain Wellness Services may be available on a limited or first-come, first‑served basis resulting from resource constraints including, but not limited to, limited staff or hours of operation, or supply shortages. HRH will communicate the resources shortage to the Employer Representative as soon as reasonably possible so that the Employer can communicate any necessary information to its employees.
Wellness Services. Of the three potential wellness services summarized in table 4: promoting physical activities, brain training and meditation, promoting physical activities and brain training services were deemed useful and desirable by potential users at all three ▇▇▇▇▇▇ pilot sites: VIVA, Orbis elderly home and Orbis care apartments. Physical activities and “brain training” take priority since they were cited in all three user contexts. Furthermore it is more desirable to promote physical activities with outdoor activities (walking, etc.) and, preferably, also linked with social activities. For this reason, there is more value added in using the agenda service (care and guidance services) for promoting physical activit ies. Instead of “brain training”, the project developed a strategy for “brain wellness training”. This strategy includes specific techniques and clinical goals: 1) Implementation intentions technique, and 2) mental wellness coaching. Further details of this service are reported in D3.1a “Design and Specification of ICT-based services”. Relaxation and meditation services were mainly cited by VIVA pilot users and were not replicated across all three user contexts. Therefore this wellness service is not a priority. Nonetheless, since NetUnion has some audio recordings (MP3) of basic mindfulness meditation (“grain of raisin”) and progressive relaxation exercises (▇▇▇▇▇▇▇▇), available in both French and Dutch; this material is available for use in the ▇▇▇▇▇▇ project. 4.1 What does Brain-Wellness mean in the ▇▇▇▇▇▇ project? Wellness services are an integral part of the ▇▇▇▇▇▇ project. However, what was expected from them, in comparison to Guidance and Care services, was less clearly defined. End users requested cognitive training games in order to maintain and improve their cognitive capabilities. The ▇▇▇▇▇▇ target group is healthy elderly living in autonomy at home. They can suffer from some slight cognitive difficulties, but they do not have important deficits. Accordingly, the brain-wellness service in the ▇▇▇▇▇▇ project is helping people to maintain a good level of cognitive functioning while giving them the opportunity to develop new abilities – and not compensate for massive deficits. A lot of elderly living alone express a lack of purpose in life or a daily routine that drives them to boredom and cognitive laziness. This leads to a subjective embarrassment. Wellness Services. LGMG shall provide or arrange to provide for wellness services as mutually agreed by the Parties, which services shall be in addition to the services set forth in Exhibit A. Such services shall be outlined in a separate written Statement of Work (“SOW”) mutually agreed to and executed by the Parties to be attached as a schedule to Exhibit A (“Wellness Services”). If any of the Wellness Services includes any clinical or medical services or oversight, such services or oversight shall be performed by applicable personnel in accordance with Applicable Law. To the extent mutually agreed to by the Parties and set forth in a SOW attached as a schedule to Exhibit A, wellness education may be provided at the Health Center or from other locations remotely, which may include, without limitation, wellness coaches or nurse educators providing the wellness services via telephone or secure video conferencing as permitted by Applicable Law. All fees and costs relating to the Wellness Services shall be negotiated in good faith between the Parties and documented in an SOW attached as a schedule to Exhibit A prior to the District incurring any costs or fees related thereto.
