Tuberculosis (TB) Testing. In the event that the services required under this Agreement may be performed within an institution/facility, office or community-based program, Contractor and its providers who are assigned to work with, near, or around CDCR/CCHCS patients and/or DJJ youth shall be required to be examined and tested or medically evaluated by a licensed health care provider for TB in an infectious or contagious stage, prior to the performance of contracted duties and at least once a year thereafter (within twelve (12) months of their initial or previous TB test under this contract), or more often as directed by CDCR/CCHCS. Contractor and its providers who may have any contact (physical or non-physical) with CDCR/CCHCS patients and/or DJJ youth, shall be required to furnish to CDCR/CCHCS, at no cost to CDCR/CCHCS, a documented TB evaluation/test for TB infection (Tuberculin Skin Test or a blood test, Interferon Gamma Release Assay) using Tuberculin Skin Test and Evaluation form (CDCR 7336), that has been completed within the thirty (30) days prior to the commencement of the contracted duties associated with this Agreement. Each evaluation/test must certify that Contractor and its providers are free of TB in an infectious or contagious stage by a licensed health care provider, prior to assuming their contracted duties. The Contractor is required to provide an updated Tuberculin Skin Test and Evaluation form (CDCR 7336) for each provider annually thereafter for the duration of this Agreement.
