Terms & Conditions. 1. The employee shall give written notice on or before April 1 of the school year prior to the fiscal year during which benefits are to begin. If at any time, more than one payout option is provided within this plan, this notice date will become December 31 of the calendar year prior to the fiscal year during which benefits are to begin.
