Teacher Requested. Leave of absence for the duration of the disability for a period up to six (6) consecutive months shall be granted to any permanent teacher who, as a result of an extended illness or injury including pregnancy or childbirth related disabilities, has exhausted their accumulation of sick leave. Upon the request of such teachers, such leave may be extended up to a total maximum of one (1) year. In all cases a physician's statement indicating diagnosis, prognosis and estimated length of disability is required before such leave is granted. A teacher returning from teacher- requested medical leave shall be reinstated to their original job or to a position of like status, pay and seniority.
