Survivor’s Benefit. In the event of your death before the end of the Term, or during the course of any applicable Severance Period, your spouse, if any, shall receive the full (or, if your death occurs during any applicable Severance Period, the remainder of the) period of continued base salary and medical and dental coverage set forth in paragraph 6 or 7 above. Further, your spouse, if any, shall receive the Guaranteed Minimum Bonus due with respect to the year of your death under paragraph 4 above.
Survivor’s Benefit. Effective January 1, 2001, or as soon thereafter as PERS requirements can be satisfied, the retirement benefits shall include Level IV of 1959 Survivor Benefits under Section 21574 of the California Government Code and Post-Retirement Survivor Allowance to Continue After Remarriage under Section 21635.5 of the California Government Code.
Survivor’s Benefit. (a) If Executive dies prior to a Separation from Service, Executive’s Beneficiary shall be entitled to the Account Balance, which became 100% vested upon the Executive's death, payable in a single lump sum on the Benefit Eligibility Date specified in Section 1.6(b).

Survivor’s Benefit. The City provides the Level 4 of the Level 1959 Survivor's Benefit for all employees enrolled in the PERS. The cost to employees for this benefit will be $2.00 per month.
Survivor’s Benefit. The City shall provide CalPERS ‘59 Survivor's Benefit fourth level coverage. This benefit provides a monthly allowance to survivors of a member who dies prior to retirement.
Survivor’s Benefit. 1. If a person specified in Article 3, was subject to the legislation of Japan at the time of his death and had previously completed a period of coverage of at least twelve months under the legislation of the Netherlands on survivors’ benefit, his survivors shall be entitled to a benefit under the latter legislation, calculated in accordance with paragraph 2 of this Article.
Survivor’s Benefit. The City shall pay the required contributions, on behalf of each unit member, to continue to provide Level IV of the 1959 Widow's Survivors Benefit.
Survivor’s Benefit. I understand that if I die prior to a Separation from Service, my designated Beneficiary shall be entitled to a Survivor’s Benefit equal to my Normal Retirement Benefit, payable in 180 monthly installments commencing on my Benefit Eligibility Date. My Benefit Eligibility Date is the first day of the month which is no less than 15 and no more than 45 days following my death.
Survivor’s Benefit. The designated survivor of a professional educator who dies while under contract with Xxxxxxxxx Public Schools will receive the benefits of sick leave. The professional educator must provide the school with a certified statement of beneficiary. The document will be included in the personnel file of the certified employee and it is the responsibility of the employee to request and complete a new form for any changes. BENEFIT REQUEST The benefits of sick leave pay must be applied for on or before the Wednesday before the last board meeting in June. Persons resigning or retiring after this date forfeit claims to benefit pay. SICK LEAVE SHARING PROGRAMS The sick leave sharing program for Xxxxxxxxx Public School Employees will become operational beginning in September 1, 2002.
