Sunscreen. I authorize SELC and staff to apply Sunscreen.
Sunscreen. Employees shall have access to SPF 30+ broad spectrum sunscreen. Employees shall, prior to commencing work in the open, apply the sunscreen to all exposed areas of their body, including lips. The sunscreen shall be reapplied during the provided rest interval/break and again during their meal break. Employee/s requiring a particular type of sunscreen because of medical reasons shall provide appropriate medical proof of such reasons to the Employer or, relevant safety supervisor whereby the particular type of sunscreen shall be subsequently provided.
Sunscreen. The CITY will purchase, in bulk, sunscreen and provide such sunscreen on an as needed basis to all CITY employees who perform work outdoors.
Sunscreen. Employees w orking out side are provided w ith sunscreen lotion (minimum SPF 30) in quantities deemed necessary for the performance of their duties.

Bug repellent Other (list any other approved over-the-counter drugs) Youth Program staff reserves the right to use generic equivalents when available for the name brand over-the-counter medications listed above. I understand that such administration will not be done under the supervision of medical personnel. I also agree that any first aid treatment may be given as needed. Any condition which is associated with fever, significant inflammation, and/or does not respond to the above outlined treatment will be followed up by a consultation with the camper’s parents. Parent/guardian will be contacted if any conditions develop requiring treatment with any of the above over-the-counter medications that are not checked. I understand that these over-the-counter medications are not necessarily kept on hand and available to be administered immediately. I authorize the administration of over-the-counter medications to my child as indicated above. I shall indemnify and hold harmless the Institution, its governing board, officers, employees, and representatives against any claims that may arise relating to my child being administered the above indicated over-the-counter medications.

Sunscreen. We recommend that a long-wearing sunscreen be applied at home before coming to school. Due to interactions and allergies, we can not apply individual sunscreen daily unless required by a child’s physician for medical reasons.

Sunscreen. A yearly sunscreen fee of $20 is charged at the beginning of May. Parents must apply sunscreen between the months of April – September, before children arrive at school. If a child arrives without sunscreen, it will be applied and a $5.00 charge will be assessed. DLC will reapply sunscreen after lunch and as needed. Water Bottles: Children must bring a water bottle to school every day. In the event that your child does not bring a water bottle, DLC will supply one and a $1.00 fee will be added to your account. Face Masks: While under health department directives for mitigating the spread of COVID-19, parents must send 2-3 face masks to school daily. If your child does not have a face mask, we will issue them one for a fee of $1 per mask. Masks are worn while indoors.
