Subscriber Agreement. COPAYMENT means either a defined dollar amount or a percentage of our allowance that you must pay for certain covered health care services.

The Summary of Pharmacy Benefits table, below, indicates the tier structure and the amount that you are responsible to pay. The tier placement of our formulary is subject to change. Formulary Listing Our formulary lists generic, preferred brand name, and non-preferred brand name prescription drugs and specialty prescription drugs covered under this agreement. Medication Synchronization (less than a 30 day supply) In accordance with Rhode Island General Law §27-18-50.1, a prorated copayment may be applied for covered prescription drugs, used to treat chronic long-term conditions, when prescribed for less than a (30) thirty day supply and dispensed by a network pharmacy if: • the prescribing physician and pharmacist determine it is in the best interest of the member; and • the member requests or agrees to less than a thirty (30) day supply. In addition, in order to qualify for medication synchronization, the covered prescription drug must: • be a maintenance drug used for the management and treatment of a chronic long- term care condition; • not be a controlled substance; • meet all utilization management requirements specific to the drug; • be of a formulation able to be split over the required shortened supply...
Subscriber Agreement. 2.1 By registering as an illion Tenancy Subscriber, you agree to follow and be bound by this Subscriber Agreement, which comprises this Subscriber Agreement, our Website Terms of Use, our Privacy Policy, any additional terms and conditions published on the illion Website in respect of the illion Tenancy Services and our procedures and any other instructions we provide when you use any illion Tenancy Service. This Subscriber Agreement applies to all services that illion Tenancy supplies to you.
Subscriber Agreement. Comodo reserves the right to change, alter, modify, update or amend the Subscriber Agreement at any time and for any reason and may do so without providing notice to Company. Any such changes will be automatically incorporated into the Certificate Manager software and presented upon the ordering of any additional Certificates by Company. Any amendments made to a Subscriber Agreement shall be binding and immediately effective upon Comodo’s posting the modified Subscriber Agreement to its website. In the event that a modification to a Subscriber Agreement materially affects Company’s ability to use the Certificate or affects Company’s business operations, Company may terminate this Agreement in accordance with Section 8.

Subscriber Agreement. The Agreement, consisting of the Order Form, this Enterprise Certificate Agreement, and any present or future Order Forms or Addendum executed by the parties, as well as any documents referenced and incorporated therein, is the subscriber agreement required by the CPS for all Publicly-Trusted Certificates issued to Subscriber and applies to multiple future Certificates requested or issued during the Term.
