Specialty Pays. 1. Employees covered by this Agreement who are assigned to and perform the duties of Bike Patrol, Field Training Officer (FTO), Police Mountain Bike Instructor, All Terrain Vehicle (Quad or Enduro) Instructor, Defensive Tactics Instructor, NCIC Instructor, or Firearms Instructor receive additional pay at the rate of four percent (4%) of their base salary in addition to their scheduled rate of pay. This additional compensation will only be paid for shifts worked by an employee when performing the specialized duties, as assigned by the Department Head or designee. The department head will establish criteria to evaluate and select employees assigned to perform the duties of Bike Patrol, which shall be for a minimum of one (1) year. Only one Park Police Officer can hold this position at one time. Upon the recommendation of his/her department head and the approval of the County Manager, an employee will be eligible to receive Bilingual Pay provided the following conditions are met: The employee’s assigned duties require them to communicate in a second language a minimum of 15% of their time; and As a prerequisite to receiving Bilingual Pay, the employee must successfully complete the County’s Bilingual Oral Proficiency Examination. The need for a written proficiency examination will be determined by County Management on a case-by-case basis. Competency testing requires fluency in English and the required foreign language or languages. The parties further recognize and agree that: Award of bilingual pay to an employee will not occur simply because the employee is bilingual and occasionally uses bilingual skills in the course of their work; Positions in which the use of a second language is a requirement are not eligible for bilingual pay; Bilingual testing will be scheduled by the County; Bilingual premium pay shall be $75.00 per pay period in a stipend form for each employee determined to be eligible pursuant to Section 3 herein. When an employee begins or ends eligibility for bilingual pay in the middle of a pay period, the stipend will be prorated.   • Dental Care a. Dental Care for Members over age 19 is limited to the following:

  • Medical Expenses 1. Employees exposed to hazardous physical, biological, or chemical agents shall be provided, at no cost to the employee, with medical examinations or evaluations required by VOSHA regulations. If there are no specific VOSHA regulations or standards for the agent in question, recommendations of the National Institute of Occupational Safety and Health or other generally recognized expert organization shall be used, as determined by the Commissioner of Health.

  • Dental Services The following dental services are not covered, except as described under Dental Services in Section 3: • Dental injuries incurred as a result of biting or chewing. • General dental services including, but not limited to, extractions including full mouth extractions, prostheses, braces, operative restorations, fillings, frenectomies, medical or surgical treatment of dental caries, gingivitis, gingivectomy, impactions, periodontal surgery, non-surgical treatment of temporomandibular joint dysfunctions, including appliances or restorations necessary to increase vertical dimensions or to restore the occlusion. • Panorex x-rays or dental x-rays. • Orthodontic services, even if related to a covered surgery. • Dental appliances or devices. • Preparation of the mouth for dentures and dental or oral surgeries such as, but not limited to, the following: o apicoectomy, per tooth, first root; o alveolectomy including curettage of osteitis or sequestrectomy; o alveoloplasty, each quadrant; o complete surgical removal of inaccessible impacted mandibular tooth mesial surface; o excision of feberous tuberosities; o excision of hyperplastic alveolar mucosa, each quadrant; o operculectomy excision periocoronal tissues; o removal of partially bony impacted tooth; o removal of completely bony impacted tooth, with or without unusual surgical complications; o surgical removal of partial bony impaction; o surgical removal of impacted maxillary tooth; o surgical removal of residual tooth roots; and o vestibuloplasty with skin/mucosal graft and lowering the floor of the mouth. Dialysis Services • The following dialysis services received in your home: o installing or modifying of electric power, water and sanitary disposal or charges for these services; o moving expenses for relocating the machine; o installation expenses not necessary to operate the machine; and o training in the operation of the dialysis machine when the training in the operation of the dialysis machine is billed as a separate service. • Dialysis services received in a physician’s office.

  • Long Term Care Insurance The University offers full-time faculty the opportunity to purchase Long-Term Care Insurance through a voluntary Long-Term Care Insurance policy. Faculty members are responsible for 100% of the premium, which may be remitted through payroll deduction.

  • Leave for Medical and Dental Care Where it is not possible to schedule medical and/or dental appointments outside regularly scheduled working hours, reasonable time off for medical and dental appointments for employees or for dependent children shall be permitted, but where any such absence exceeds two (2) hours, the full-time absence shall be charged to the entitlement described in Clause 20.13. "Medical and/or dental appointments" include only those services covered by the B.C. Medical Services Plan, the Employer's Dental Plan, the Extended Health Benefit Plan and appointments with the Employee and Family Assistance Program.

  • Pharmacy Services The Contractor shall establish a network of pharmacies. The Contractor or its PBM must provide at least two (2) pharmacy providers within thirty (30) miles or thirty (30) minutes from a member’s residence in each county, as well as at least two (2) durable medical equipment providers in each county or contiguous county.

  • Medical, Dental and Vision Insurance The Employer provides health care coverage for medical, dental and vision for the employee, spouse and children (one-person, two-person and family) subject to the provisions of this Article. Currently the plan options are:
