Common use of Special Reporting Requirements Clause in Contracts

Special Reporting Requirements. (1) All A&D 80 Services provided by County under this Agreement must be reported and submitted electronically to OHA on a quarterly basis through the Oregon Problem Gambling Prevention Quarterly Data Reporting Collection System, located at xxxxx://xxx.xxxxxx.xxx/oha/HSD/Problem- Gambling/Pages/Prevention.aspx no later than 45 calendar days following the end of each quarter February, May, August, November and February with respect to Services provided in the prior quarter.
