Common use of Social Services Clause in Contracts

Social Services. Social services shall be provided by an individual who possesses at least a baccalaureate degree in social work from a college or university or a baccalaureate degree in a human services field such as sociology, special education, rehabi- litation, counseling or psychology and shall be available to the extent necessary to meet the needs of patients and their families for care that is reasonable and necessary for the palliation and management of terminal illness and related conditions. An indi- vidual providing social services who does not possess the appro- priate degree as of June 1, 1992 shall have 5 years after June 1, 1992 to obtain the appropriate degree. The duties of the individual providing social services shall include the following: 1. Regularly assessing the patient’s social service needs, implementing the plan of care provisions to meet those needs and reevaluating the patient’s needs and providing ongoing psychoso- cial assessment of the family’s coping capacity relative to the patient’s terminal condition; and 2. Linking patient and family with needed community resources to meet ongoing social, emotional and economic needs;
