Upon satisfactory written verification from a medical physician of such medical condition, the employee will be entitled to such accommodation as the Employer may direct or failing that, access to the sick leave plan, if any, during any outbreak period. If an employee did not take the required annual immunization vaccination or complete a course of treatment initiated by the Employer, any subsequent course of treatment taken during an influenza outbreak or otherwise shall be undertaken at the employee’s expense. Sick Leave Certificate. The Employer may request a medical certificate verifying an absence of three (3) or more working days. Such certificate will be signed by the attending physician indicating that the Employee was seen or attended by that Physician.

Sick Leave Certificate. Any employee returning from any absence of four (4) or more days due to illness or injury is required to produce at his or her own expense a certificate of a licensed physician attesting to his or her fitness for work, before resuming work. If the employee is unable to return to work after four (4) days, he or she shall submit a certificate of a licensed physician at the first practical opportunity indicating the nature of the illness or injury and a prognosis as to the likely date of return to work, if known.
