Services Contractor Agrees to Perform. Contractor agrees to perform the Services provided for in Appendix A, “Scope of Services." Officers and employees of the City are not authorized to request, and the City is not required to reimburse the Contractor for, Services beyond the Scope of Services listed in Appendix A, unless Appendix A is modified as provided in Section 11.5, "Modification of this Agreement."
Services Contractor Agrees to Perform. The Contractor agrees to perform the services provided for in Appendix A, “Description of Services,” attached hereto and incorporated by reference as though fully set forth herein.

If Appendix A includes as-needed services, such services shall be requested by City through the issuance of a written task order signed by City and Contractor, which task order shall be made a part of and incorporated into the Agreement as though fully set forth herein without the need for a formal amendment to the Agreement. The task order shall include a description of the as-needed services, the deliverables, schedule for performance, cost, and method and timing of payment.
Services Contractor Agrees to Perform. During the Term of this Agreement, Contractor will perform all of the services set forth in Appendix A (SaaS Implementation and Training Services), Appendix B (SaaS Application and Hosted Services), and the following:

The Contractor shall provide monthly preventative maintenance, routine repair and 5-year load tests, as needed for San Francisco Unified School District’s elevators, dumbwaiters, chairlifts and stairlifts per City of Kansas City, Missouri, (“Lead Public Agency”), on behalf U.S. Communities Government Purchasing Alliance, which is now OMNIA Partners, Contract No. EV2516 received and opened on June 8, 2018 consisting of signed Purchase Agreement Proposal, General Terms, Conditions, Instructions and Information for Bidders, Special Conditions and Bid Sheet. In addition, the Contractor shall perform the works as described in Attachment A of this contract.

Contractor shall adhere to the quality assurance guidelines set forth in Appendix C, “SFMTA Contractor Checklist for Document Submittals,” and respond fully and promptly to requests for revisions to initial Submittals from the City in order to finalize documents.
