Service Capacity. 5.7.1. Using the form provided by Great Rivers, the Contractor shall submit to Great Rivers a roster of all clinical staff. The roster shall be updated monthly and updates to the roster are due by the 10th of each month. The roster should identify each staff member’s name, credentials, role/job title, working locations, contact information, language, if accepting new enrollees and if the staff member is a specialist as defined by WAC. If there are providers who can be accessed on a case by case basis, information on how to access these providers must be included.
