Semi-Private Hospital Insurance. The Hospital agrees to pay seventy-five (75%) of the billed premiums towards coverage of eligible employees in the active employ of the Hospital under the Liberty Health Plan or comparable coverage with another carrier.
Semi-Private Hospital Insurance. The Hospital agrees to pay 100% of the billed premiums towards coverage of eligible employees in the active employ of the Hospital under the Green Shield Health Plan or comparable coverage with another carrier.
Semi-Private Hospital Insurance unlimited - covers the difference between xxxx and semi-private. Semi-Private Hospital Insurance is provided and is a condition of employment for all eligible employees. The Board will participate in the cost of single or family coverage as requested by the employee at a) 90% for a full-time employee, and b) 40% for a part-time employee.

Extended Health Care The Hospital shall contribute on behalf of each eligible employee seventy-five percent (75%) of the billed premium under the Extended Health Care Plan (Blue Cross deductible plan including hearing aids with a maximum of per person and vision care with a maximum of every months per person, or its equivalent) provided the balance of the monthly premium is paid by employees through payroll deduction. Any Hospital currently paying more than of the premium shall continue to do so. The drug formulary shall be as defined by Liberty Health Formulary Three.
Semi-Private Hospital Insurance. The Hospital agrees to pay of the billed premiums coverage of eligible employees in the active employ of the Hospital under the Blue Cross Semi-Private Plan or cornparable coverage with another carrier. Extended Health Effective the first of the month following the date of the award the Hospital shall contribute on behalf of each eligible employee seventy-five percent of the billed premium under the Extended Health Care Plan (Blue Cross plan including hearing aids with a maximum of per person and vision care with a maximum of every months per person, or its equivalent) provided the balance of the monthly premium is paid by employees through payroll deduction (subject to appropriate Union and employer recruitment). Any Hospital currently paying more than of the premium shall continue to do so. Effective April deductible for the Extended Health Care Plan will be (single) (family) and the vision care maximum will to every months per person.
