Role of Health Service Provider. The main role of the Centre under this Agreement is to provide the services detailed in the schedules contained in clause 13. The delivery of the services must be in accordance with the performance measurers and targets set by the Department CEO in accordance with section 46(3)(d),(e) and (f) of the Act. The Centre will deliver the services in accordance with the Agreement. This includes, but is not limited to:  delivering the services in a safe and timely and efficient manner using the standard of care and foresight expected of an experienced provider  acting in accordance with the highest applicable professional ethics, principles and standards  demonstrating a commitment to ethical practices and behaviours, and implementing these practices through appropriate training and monitoring. When delivering the services, the Centre is required to comply with (among other things):  all standards as gazetted under applicable Acts and standards endorsed by the Department CEO  all applicable Department Policy Frameworks  performance targets (as referred to in clause 9 of this Agreement)  laws including those related to fire protection, industrial relations, employment, health, general safety and taxation.

Role of Health Service Provider. The main role of the EMHS under this Agreement is to provide the health services detailed in the Schedules, as well as TTR in support of the provision of health services. The EMHS is responsible for providing health services at the following facilities: • Royal Perth Hospital • Bentley Hospital • Kalamunda Hospital • Armadale-Kelmscott Memorial Hospital The EMHS manages the contract with St Xxxx of God Health Care for the operation of St Xxxx of God Midland Public Hospital. The EMHS also manages a separate contract with St Xxxx of God Mt Xxxxxx for specific health services. The EMHS will deliver the health services in accordance with this Agreement. This includes, but is not limited to: • delivering the health services in a safe, timely and efficient manner using the standard of care and foresight expected of an experienced provider • acting in accordance with the highest applicable professional ethics, principles and standards • demonstrating a commitment to ethical practices and behaviours, and implementing these practices through appropriate training and monitoring. Role of Health Service Provider. The main role of the NMHS under this Agreement is to provide the health services detailed in the Schedules, as well as TTR in support of the provision of health services. The delivery of the health services must be in accordance with the performance measures and targets set by the Department CEO in accordance with section 46(3)(d), (e) and (f) of the Act. The NMHS is responsible for providing health services at the following facilities: • Graylands Hospital • Joondalup Health Campus • Xxxx Xxxxxx Memorial Hospital • Xxxxxxx Park Hospital • Xxx Xxxxxxx Xxxxxxxx Hospital The NMHS is also responsible for providing dental health services through the Dental Health Service and managing the contract with Ramsay Health Care for the operation of the Joondalup Health Campus. The NMHS will deliver the health services in accordance with this Agreement. This includes, but is not limited to: • delivering the health services in a safe, timely and efficient manner using the standard of care and foresight expected of an experienced provider • acting in accordance with the highest applicable professional ethics, principles and standards • demonstrating a commitment to ethical practices and behaviours, and implementing these practices through appropriate training and monitoring. When delivering the health services, the NMHS is required to comply with (among other things): • all standards as gazetted under applicable Acts and standards endorsed by the Department CEO • all applicable Department Policy Frameworks • performance targets (as referred to in clause 10 of this Agreement) • laws including those related to fire protection, industrial relations, employment, health, general safety, and taxation.

The SMHS is responsible for providing health services at the following facilities: • Xxxxx Xxxxxxx Hospital • Fremantle Hospital • Xxxxxx District Hospital • Rockingham General Hospital • State Rehabilitation Centre The SMHS also manages the contract with Ramsay Health Care for the operation of Peel Health Campus. The SMHS will deliver the health services in accordance with this Agreement. This includes, but is not limited to: • delivering the health services in a safe, timely and efficient manner using the standard of care and foresight expected of an experienced provider • acting in accordance with the highest applicable professional ethics, principles and standards • demonstrating a commitment to ethical practices and behaviours, and implementing these practices through appropriate training and monitoring. When delivering the health services, the SMHS is required to comply with (among other things): • all standards as gazetted under applicable Acts and standards endorsed by the Department CEO • all applicable Department Policy Frameworks • performance targets (as referred to in clause 10 of this Agreement) • laws including those related to fire protection, industrial relations, employment, health, general safety, and taxation.

The XXXXX is responsible for providing health services at the following regional facilities:  Albany Hospital  Broome Hospital  Bunbury Hospital  Geraldton Hospital  Hedland Health Campus  Kalgoorlie Hospital The XXXXX is responsible for providing health services at the following district facilities:  Busselton Hospital  Collie Hospital  Carnarvon Hospital  Derby Hospital  Esperance Hospital  Katanning Hospital  Kununurra Hospital  Xxxxxxxx River Hospital  Merredin Hospital  Moora Hospital  Narrogin Hospital  Xxxxxx Hospital  Xxxxxx Bay Hospital (Karratha)  Northam Hospital  Xxxxxx Hospital (Manjimup) The XXXXX will deliver the services in accordance with the Agreement. This includes, but is not limited to:  delivering the services in a safe and timely and efficient manner using the standard of care and foresight expected of an experienced provider  acting in accordance with the highest applicable professional ethics, principles and standards  Demonstrating a commitment to ethical practices and behaviours, and implementing these practices through appropriate training and monitoring. When delivering the services, the XXXXX is required to comply with (among other things):  all standards as gazetted under applicable Acts and standards endorsed by the Department CEO  all applicable Department Policy Frameworks  performance targets (as referred to in clause 9 of this Agreement)  laws including those related to fire protection, industrial relations, employment, health, general safety and taxation.
Role of Health Service Provider. The main role of the CAHS under this Agreement is to provide the services detailed in the schedules contained in clause 13, as well as teaching, training and research in support of the provision of health services. The delivery of the services must be in accordance with the performance measurers and targets set by the Department CEO in accordance with section 46(3)(d),(e) and (f) of the Act. The CAHS has the control and management of the following facilities:  Princess Xxxxxxxx Hospital to the date that the final patient moves from the Princess Xxxxxxxx Hospital.  Perth Children’s Hospital from the date that the first patient arrives at the Perth Children’s Hospital. The CAHS is responsible for providing health services at the following facilities:  Princess Xxxxxxxx Hospital to the date that the final patient moves from the Princess Xxxxxxxx Hospital.  Perth Children’s Hospital from the date that the first patient arrives at Perth Children’s Hospital. The CAHS will deliver the services in accordance with the Agreement. This includes, but is not limited to:  delivering the services in a safe and timely and efficient manner using the standard of care and foresight expected of an experienced provider  acting in accordance with the highest applicable professional ethics, principles and standards  demonstrating a commitment to ethical practices and behaviours, and implementing these practices through appropriate training and monitoring. When delivering the services, the CAHS is required to comply with (among other things):  all standards as gazetted under applicable Acts and standards endorsed by the Department CEO  all applicable Department Policy Frameworks  performance targets (as referred to in clause 9 of this Agreement)  laws including those related to fire protection, industrial relations, employment, health, general safety and taxation.
Role of Health Service Provider. The main role of the Quadriplegic Centre under this Agreement is to provide the health services detailed in the schedules. The delivery of the health services must be in accordance with the performance measures and targets set by the Department CEO in accordance with section 46(3)(d),(e) and (f) of the Act. The Quadriplegic Centre will deliver the Health Services in accordance with this Agreement. This includes, but is not limited to: • delivering the health services in a safe, timely and efficient manner using the standard of care and foresight expected of an experienced provider • acting in accordance with the highest applicable professional ethics, principles and standards • demonstrating a commitment to ethical practices and behaviours, and implementing these practices through appropriate training and monitoring. When delivering the health services, the Quadriplegic Centre is required to comply with (among other things): • all standards as gazetted under applicable Acts and standards endorsed by the Department CEO • all applicable Department Policy Frameworks • performance targets (as referred to in clause 10 of this Agreement) • laws including those related to fire protection, industrial relations, employment, health, general safety, and taxation.

This includes, but is not limited to:  delivering the health services in a safe, timely and efficient manner using the standard of care and foresight expected of an experienced provider  acting in accordance with the highest applicable professional ethics, principles and standards  demonstrating a commitment to ethical practices and behaviours, and implementing these practices through appropriate training and monitoring. When delivering health services, CAHS is required to comply with (among other things):  all standards as gazetted under applicable Acts and standards endorsed by the Department CEO  all applicable Department Policy Frameworks  performance targets (as referred to in clause 10 of this Agreement)  laws including those related to fire protection, industrial relations, employment, health, general safety, and taxation.

Related to Role of Health Service Provider

  • Mental Health Services This agreement covers medically necessary services for the treatment of mental health disorders in a general or specialty hospital or outpatient facilities that are: • reviewed and approved by us; and • licensed under the laws of the State of Rhode Island or by the state in which the facility is located as a general or specialty hospital or outpatient facility. We review network and non-network programs, hospitals and inpatient facilities, and the specific services provided to decide whether a preauthorization, hospital or inpatient facility, or specific services rendered meets our program requirements, content and criteria. If our program content and criteria are not met, the services are not covered under this agreement. Our program content and criteria are defined below.

  • Health Services At the time of employment and subject to (b) above, full credit for registered professional nursing experience in a school program shall be given. Full credit for registered professional nursing experience may be given, subject to approval by the Human Resources Division. Non-degree nurses shall be placed on the BA Track of the Teachers Salary Schedule and shall be ineligible for movement to any other track.

  • Behavioral Health Services Behavioral health services include the evaluation, management, and treatment for a mental health or substance use disorder condition. For the purpose of this plan, substance use disorder does not include addiction to or abuse of tobacco and/or caffeine. Mental health or substance use disorders are those that are listed in the most updated volume of either: • the Diagnostic and Statistical Manual of Mental Disorders (DSM) published by the American Psychiatric Association; or • the International Classification of Disease Manual (ICD) published by the World Health Organization. This plan provides parity in benefits for behavioral healthcare services. Please see Section 10 for additional information regarding behavioral healthcare parity. Inpatient This plan covers behavioral health services if you are inpatient at a general or specialty hospital. See Inpatient Services in Section 3 for additional information. Residential Treatment Facility This plan covers services at behavioral health residential treatment facilities, which provide: • clinical treatment; • medication evaluation management; and • 24-hour on site availability of health professional staff, as required by licensing regulations. Intermediate Care Services This plan covers intermediate care services, which are facility-based programs that are: • more intensive than traditional outpatient services; • less intensive than 24-hour inpatient hospital or residential treatment facility services; and • used as a step down from a higher level of care; or • used a step-up from standard care level of care. Intermediate care services include the following: • Partial Hospital Program (PHP) – PHPs are structured and medically supervised day, evening, or nighttime treatment programs providing individualized treatment plans. A PHP typically runs for five hours a day, five days per week. • Intensive Outpatient Program (IOP) – An IOP provides substantial clinical support for patients who are either in transition from a higher level of care or at risk for admission to a higher level of care. An IOP typically runs for three hours per day, three days per week.

  • Extended Health Care Plan (a) The Employer shall pay the monthly premium for regular employees entitled to coverage under a mutually acceptable Extended Health Care Plan.

  • Health & Safety (a) The Employer and the Union agree that they mutually desire to maintain standards of safety and health in the Home, in order to prevent injury and illness and abide by the Occupational Health and Safety Act as amended from time to time.

  • Citizen Volunteer or Community Service Leave Leave without pay may be granted for community volunteerism or service.

  • Certain Employees (a) Each of the following is included in the list of agreements set forth in the Disclosure Schedule: all collective bargaining agreements, employment and consulting agreements, bonus plans, deferred compensation plans, employee pension plans or retirement plans, employee profit-sharing plans, employee stock purchase and stock option plans, hospitalization insurance, and other plans and arrangements providing for employee benefits of employees of the Seller.

  • Occupational Health & Safety (a) It is a mutual interest of the parties to promote health and safety in workplaces and to prevent and reduce the occurrence of workplace injuries and occupational diseases. The parties agree that health and safety is of the utmost importance and agree to promote health and safety and wellness throughout the organization. The employer shall provide orientation and training in health and safety to new and current employees on an ongoing basis, and employees shall attend required health and safety training sessions. Accordingly, the parties fully endorse the responsibilities of employer and employee under the Occupational Health and Safety Act, making particular reference to the following:

  • Home Health Care This plan covers the following home care services when provided by a certified home healthcare agency: • nursing services; • services of a home health aide; • visits from a social worker; • medical supplies; and • physical, occupational and speech therapy.

  • Special Service networks The following services must be received from special service network providers in order to be covered. All terms and conditions outlined in the Summary of Benefits apply.
