As the lead organization, NIJC is responsible for the following: • Ensuring the activities and finances of the NACTF project are in compliance with CalOES requirements and relevant regulations. • Serving as the administrator and implementer of all NACTF project activities as outlined above. • Providing transition support to advise PARTNERS on the installation, maintenance, upgrades and sustainment of resources provided to improve IT capacity. • Coordinating an Expert Panel to guide the project and communicate needs from the field to the project • Reporting NACTF activities and accomplishments to CalOES. Under this Agreement, PARTNER agrees to: • Provide goals, objectives and anticipated outcomes for the IT resources, including targeted number of people served, as described in Attachment A, and report on them to NIJC. • Assign a liaison to coordinate with NIJC in the identification, installation, maintenance, and sustainability of IT resources at the Partner site. • Promptly respond to document and information requests to receive resources (failure to promptly respond could result in loss of resources). • Participate in a pre- and end-of-project assessment which will identify project targets or goals for the new IT resources. • Participate in 1 or more of the project’s blended trainings either in person or online. • Request technical assistance from NIJC in a timely manner for help in implementing resources or participating in project. • Comply with all appropriate local, state or Federal laws and regulations regarding the use of resources provided through NACTF; • Participate in all evaluation activities required by agencies funding this project; and • As a regional hub site XXXX will work with NIJC project staff to organize and promote a distance learning training session for 10 or more personnel from tribal programs in their regions using technology equipment and an existing project Moodle course. The hub site will send 3 staff members to a training-of-trainers session at NIJC’s offices in Santa Xxxx. The session will consist of a one day rehearsal on course delivery methods and a half-day to one day of conducting the course for the 10 personnel in another location. Responsibilities and Expectations. For the campaign, the Michigan Nonprofit Association (MNA) serves as the "lead" organization. As the lead organization, MNA is responsible for the following: • Secure and distribute funding to for distribution of mini-grants in the region. • Develop an evaluation plan, including a dashboard for Community Foundation X to track its efforts and those of their mini-grant recipients. • Design and implement a statewide communications strategy that includes a plan for the region (website, media outreach, talking points, ads, etc). • Provide Community Foundation X with the data to identify hard-to-count neighborhoods in the areas referenced and ideas for outreach. • Provide tools that can be tailored, by Community Foundation X, on the planning and implementation for census 2020; what’s at stake for communities; examples for how nonprofits can be engaged; responses to FAQs; and sample materials to be provided to those who are hardest-to-count. • Coordinate with state and federal officials to collaborate on efforts to reach those who are hardest-to-count and avoid duplication of efforts. MNA will share this information with the census hubs. • Host quarterly online trainings for mini-grant recipients. • Conduct monthly peer learning calls with census hub representatives to discuss the latest regarding campaign efforts statewide, promote leading practices and sharing of strategies. • Provide ongoing technical assistance to census hub representatives. Under this Agreement, Community Foundation X agrees to: • Form an advisory group comprised of representatives from the targeted areas to advise on the region’s plans for outreach, training, communications and mini- grant distribution. • Develop plans for the region that is based on the data provided by MNA to reach the hardest-to-count neighborhoods. • With input from the advisory group and MNA, identify nonprofits that serve those in the targeted hardest-to-count neighborhoods. As the lead organization, Middlesex-London Health Unit is responsible for the following:  Will be expected to train staff (or a least 2 train the trainers) from eligible organizations on who may receive naloxone and the recommended quantity to dispense. Trainers will then be responsible for training new staff at their organization.  Train staff at eligible organizations on how to administer naloxone in cases of opioid overdose, recognizing the signs of overdose and ways to reduce the risk for overdose. Responsibilities and Expectations. The PT will have to have an informal interview with a Phoenix Gym Director to ensure their suitability and availability to work within the Phoenix Gym Studio. • PT sessions are to start and finish promptly at the scheduled time. Please ensure that the Studio is vacated at the end of the session in a timely fashion ready for the next personal trainer. • Phoenix Gym agrees to maintain the PT studio. However the Studio must be risk assessed by the PT prior to starting the session. It is the PT’s duty to check all equipment is in sound working order prior to the start of the session and to report any faults in equipment to the Phoenix Gym staff. If the PT notices any equipment becomes damaged, dangerous or unusable/unsafe for continued use then the PT must take it out of action for the safety of clients and notify one of the Phoenix Gym staff. • If the PT has any useful feedback or suggestions for the improvement of the studio or operational systems of running the studio then the PT’s feedback will be very welcome. • If the tissue dispenser runs out of tissue or antibacterial spray then please notify any member of staff so that they can re-stock it. • All Studio equipment remains the property of Phoenix Gym unless the PT brings in any of their own equipment for their class. Any equipment left at Phoenix Gym is done so at the PT’s own risk. Phoenix Gym is responsible for maintaining their own equipment and the PT is responsible for maintaining their own equipment. • The PT will need to be qualified to at least a level 2 or 3 standard in Fitness Instruction or Personal Training. Boxing/martial arts Instructors must keep within their professional expertise to ensure the safety of their clients. • The PT must wear smart/professional and appropriate clothing whilst training clients. • The PT will be responsible for their client whilst they are training them within the Phoenix Gym studio and make sure that their client follows the gym rules and adheres to any fire/ emergency procedures. • The PT will not be responsible for the gyms daily cleaning procedures unless they make unnecessary mess (bringing grass in from outside or spilling drinks on the studio flooring) but will be expected to keep the studio clean and tidy. The PT is also responsible for their client maintaining clean standards.
