REQUIRING MEDICAL. AUTHORIZATIONS - We shall require all teams participating in the tournament or games to provide notarized medical authorizations for each player in a form adequate for use at the site of the tournament. These authorizations shall be presented to the Host Organization at registration and kept at the field available for use by the team.
