Code \u00a7 5-30-5-2, as amended, and contain all of the following:\n(a) An overview and description of the Project, including:\n(1) the size and function of each facility;\n(2) the approximate budget of the Project; and\n(3) the anticipated schedule for the Project.\n(b) An overview and description of the selection process, including:\n(1) the process of communication with Owner;\n(2) the schedule for the selection process;\n(3) the procedures established for the Technical Review Committee (\u201cTRC\u201d); and\n(4) a description of submission requirements.\n(c) The general qualifications for potential design-builders, including:\n(1) experience with similar projects;\n(2) experience with the design-build process\n(3) organizational resources;\n(4) financial strength and bonding capacity;\n(5) Minority Business Enterprises and Women\u2019s Business Enterprises history;\n(6) litigation and dispute history; and\n(7) experience in dealing with bonding authorities.\n(d) The project-specific qualifications for potential design-builders, including:\n(1) team experience with the building type of the Project;\n(2) team performance record for quality, schedule and cost;\n(3) proposed team composition and experience working together;\n(4) current capacity to manage the Project;\n(5) proposed key project personnel; and\n(6) client references.\n(e) A description of the evaluation process for the qualification proposals, which must include:\n(1) an established rating system in compliance with Ind. Code \u00a7 5-30-5-4 and -5, each as amended; and\n(2) a briefing session or formal question and answer process with potential design-builders prior to the submission of the qualification proposals.\n2. Request for Verified Statement of Qualifications, which shall comply with Ind. Code \u00a7 5-30-5-3, as amended, and contain all of the following:\n(a) A list of all prime contractors and architectural and engineering firms participating financially on the team;\n(b) A statement that the design-builder or team members have completed or demonstrate the experience, competency, capability and capacity to complete projects of similar size, scope, and complexity;\n(c) A statement that key personnel have sufficient experience and training to manage and complete the design and construction of the Project;\n(d) A statement that the design-builder or team members have licenses, registrations and credentials required to design and construct the Project, including information on revocation or suspension of a license, registration or credential;\n(e) A statement that the design-builder has capacity to obtain required payment and performance bonds, liability insurance and errors and omissions insurance;\n(f) The experience modifier rate, the United States Occupational Safety and Health Administration (\u201cOSHA\u201d) total recordable case incident rate (TCIR) and days away, restricted or transferred case incident rate (DART) for the design-builder and each team, and the average OSHA TCIR and DART rates for the industrial classification of the design-builder and each team;\n(g) A statement that the design-builder or employees of the team performing construction, including the employees of all subcontractors, have completed or are enrolled in an apprenticeship program certified by the United States Department of Labor Bureau of Apprenticeship and Training;\n(h) Information regarding any prior serious, repeat, willful, or criminal violation by the design-builder or any team member of the federal Occupational Safety and Health Act of 1970 and any equivalent violation by the design-builder or any team member under a state plan authorized under Section 18 of the federal act that has become a final order;\n(i) Information concerning the debarment, disqualification or removal of the design-builder or a team member from any local government public works projects; and\n(j) Information concerning the bankruptcy or receivership of the design-builder or a team member.\nD. DCD shall allow sufficient time for review of the RFQ documents by Owner and Consultant.\n\u2587. DCD shall assist in training TRC, and if appointed by Owner serve as a member(s) of TRC. In addition, DCD shall coordinate, facilitate and document all TRC meetings.\nF. DCD shall assist TRC and Consultant in establishing an RFQ rating system, which shall comply with Ind. Code \u00a7 5-30-5-4, as amended, and shall include consideration of any of the following:\n1. The design-builder\u2019s experience;\n2. The design-builder\u2019s financial and bonding capacity;\n3. The design-builder\u2019s managerial resources and management plan;\n4. The design-builder\u2019s safety record;\n5. The design-builder\u2019s past performance and capacity to perform;\n6. The design-builder\u2019s ability to complete work in a timely and satisfactory manner; and\n7. Other selection criteria set out in the RFQ documents.\nG. DCD shall assist Owner in advertising and distribution of the RFQ documents to interested design-builders including all pre-submittal meetings.\nH.
