A Request for Placement clause defines the process by which one party formally asks another party to provide a specific service, resource, or personnel placement. Typically, this clause outlines the required information, format, and method for submitting such requests, as well as any timelines or approval procedures involved. Its core function is to establish a clear and consistent protocol for initiating placements, thereby reducing misunderstandings and ensuring that both parties are aligned on expectations and requirements.

a. This form is used to request both Group and Individual (Capstone/Leadership) experiences.
b. Timeline for requests and placements is as follows:
i. Winter/Spring Terms (starts and ends between January 1 and May 31)
1. Request Form Opens: September 1 2. Request Form Closes: October 1 3. Schools Notified of Placements: Early November
ii. Summer Term (starts and ends between May 1 and August 31)
1. Request Form Opens: February 1 2. Request Form Closes: March 1

a. Deadlines for placement are: March 15 for Summer, April 15 for Fall, and October 1 for Winter/Spring.

b. Home Health: additional items may be required with request. If requesting Home Health placements, please check with the Academic Placement Coordinator for requirements. There will be additional orientation to the home health setting upon placement.
