Request for Medical Documentation. An employee reporting sick shall be required to file a doctor’s certificate from time to time with Employee Health Services, if requested in advance. In absences of five consecutive days or in other reasonable circumstances the Administrator or designate may request submission of a Treatment Memorandum. Such certificate or Treatment Memorandum shall be paid for by the Corporation when payment is required.
Request for Medical Documentation. The Company may request a doctor's note after an employee has been off duty due to illness or injury in excess of three (3) consecutive days or where there may be suspected abuse.
