Reportable Incidents. CONTRACTOR shall report within 24 hours all incidents affecting the immediate health, safety and well being of clients to the office of the Director of Mental Health. Reportable incidents include, but are not limited to, all deaths, episodes of acute life threatening illness, serious physical or psychological injuries (or risk thereof), and allegations of abuse and/or neglect. CONTRACTOR shall establish procedures for the investigation of such incidents and shall cooperate with any additional investigation COUNTY may wish to conduct.
