Rates of Wages. Such contributions shall be made on or before the fifteenth (15th) of the month following the month in which such hours were worked and shall be accompanied by a remittance report form for such employees as prescribed by the Trustees of the applicable Fund. Each monthly report and contributions shall include all obligations arising from hours worked up to the close of the Employer’s payroll ending the nearest to the last day of the preceding calendar month. Such contributions shall be made payable by cheque to the applicable Fund. 44
