Rates and Services. Provider shall provide or arrange for the provision of services in accordance with the Mandates, as and when requested by ILS Community Network or Managed Care Plan from time to time, which may include without limitation the following, as well as the services described by Payor Contract, Waiver application, Mandates, and/or your Provider license. The Provider shall identify to ILS Community Network and the applicable Managed Care Plan if any aspect of Covered Services herein are to be subcontracted by the Provider. Provider will comply with all sections of this Agreement related to Provider subcontracting.

Rates and Services. 6.1 Paying all general and water rates and all other existing and future rates taxes duties charges assessments and outgoings whatsoever (whether parliamentary parochial local or of any other description) for the time being payable in respect of the Common Parts of the Estate
Rates and Services. In respect of the payment of rates and service charges for essential services and infrastructure, the municipality shall: a) Assist to facilitate community meetings for payment of services; b) Disseminate information regarding payment Capacity and skills development: The Municipality shall: a) provide direct technical assistance, training and capacity building to the Development Committee and other local structures including in respect of: how to run a development Committee effectively (e.g. accountability and reporting back to the Community); developmental processes; council processes; grant funding instruments; b) other training and capacity building in terms of a training plan to be developed collaboratively with the Committee (e.g. in respect of local contactors, community based maintenance etc.).
Rates and Services. The Supplier shall furnish to the Cooperative a complete schedule of its current and relevant rates and rate pricing options for Energy Service. Any subsequent change in the Supplier’s existing rate structures or rate pricing options for Energy Service shall be transmitted to the Cooperative as they occur. These notifications will include the old and new rates, old and new pricing options, and effective date for billing each rate. The Supplier shall transmit a sample xxxx calculation of a Member’s xxxx with 500 kW or other Member sample if it better fits the rate structure for each rate change. For Member inquiries pertaining to energy service, the Supplier shall transmit their current toll free telephone number and changes as they occur. These transmissions shall be in an electronic format acceptable to the Cooperative, and will be provided by the Supplier to the Cooperative as described on the “NH Electric Cooperative Consolidated Billing – Supplier Rate Sheet” in the section labeled “Consolidated Billing – Establishing Rates or Changing Rate Factors”. . Supplier rates and pricing options must conform to the rate structure in use by the Cooperative for each specific rate class and be supported by meters in place.

Minimum visit policy: Lucky Paws, LLC has a minimum visit policy to comply with our insurance policy and for the health and wellbeing of our Client’s pet(s). Client’s dog(s) must be visited a minimum of 3 times per day if Client is away. Client’s cat(s) must be visited a minimum of ONE visit per day. Visit times are approximate and subject to change based on the needs of all pets under Lucky Paws, LLC care. Lucky Paws, LLC is entrusted to use best judgment in caring for pet(s). Other services subject to a supplemental invoice include but are not limited to: Supply Pickup (food, treats, medications etc), Vet visit, After hours Emergency Vet visit, Extensive Incidental Clean-up, House Emergency(Waiting for service person), Return Check/Payment Fee, Pet Sitting/Dog Walking Late Payments
Rates and Services. (a) Tank Storage Fee. GEL shall pay to Lazarus a rate of $0.50 per shell barrel per month for the use of the Storage Tanks (the "Tank Storage Fee"), for the limited purpose of providing funds to Lazarus for start up of operations at the Facility. Lazarus shall receive the Tank Storage Fee as a part of, and the Tank Storage Fee shall be included in the calculation of the Distribution of Gross Profits (as defined in the Joint Marketing Agreement), until such time as GEL determines, in consultation with Lazarus, that the Tank Storage Fee is no longer necessary to fund Lazarus' operations at the Facility. GEL shall be reimbursed for all Tank Storage Fees paid in accordance with the terms of the Joint Marketing Agreement.

Interstate and International Long Distance Services (Cont’d) 3.2.2 Operator Services Responsibilities of the Customer for Operator Services (Cont’d) The Customer is responsible for providing the Company with a valid method of billing for each call. The Company reserves the right to validate the credit worthiness of users through available credit card, calling card, called number, third party telephone number and room number verification procedures. Where a requested billing method cannot by validated, the user may be required to provide an acceptable alternate billing method or the Company may refuse to place the call.
