This Solicitation contains a Solicitation, Offer, and Proposal Form which shall be submitted by either (choose one): • A sealed envelope, with Original (wet) signature in indelible ink and signed in the proper spaces by a person authorized to sign on behalf of the firm. Responses on Vendor forms will not be accepted. Firms electing to submit original forms shall also provide a complete copy of the bid response via flash drive. • Uploading an Excel file to Vendor Registry which includes the signed Proposal Form with Original signatures in indelible ink and signed in the proper spaces by a person authorized to sign on behalf of the firm.
