Proof of Qualifications. Humanus shall require Independent Contractor to produce a valid license or license eligibility, FBI and state background checks, child abuse clearance, and any other documentation or certification needed by Client to satisfy the requirements of their governing body. Client agrees to notify Humanus of their requirements with each referral order. Humanus agrees to provide a copy of said information to Client within 24 hours from the time of request. If an Independent Contractor subcontracts all or part of the services provided to Client, the Independent Contractor will require all subcontractors to provide the information under paragraph 5 to Humanus.

After a request is made for qualifications, Contractor shall provide proof of such qualifications no later than one (1) business day after a request is made.   • Staff Qualifications a) CONTRACTOR shall ensure that all individuals employed, contracted, and/or otherwise hired by CONTRACTOR to provide classroom and/or individualized instruction or related services hold a license, certificate, permit, or other document equivalent to that which staff in a public school are required to hold in the service rendered consistent with Education Code section 56366.1(n)(1) and are qualified pursuant to Title 5 of the California Code of Regulations sections 3064 and 3065.

  • Qualifications for Voting To be entitled to vote at any meeting of Holders a Person shall (a) be a Holder of one or more Notes on the record date pertaining to such meeting or (b) be a Person appointed by an instrument in writing as proxy by a Holder of one or more Notes on the record date pertaining to such meeting.
