PROJECT TERMS AND CONDITIONS. Recipient shall complete the Project in accordance with this Project Statement, which shall include the following attachments, each of which is incorporated by reference and made a part of this Project Statement and the Master Agreement: The PCORI-approved Project Plan attached hereto as Attachment B; The PCORI-approved Project Milestone Schedule attached hereto as Attachment C; The PCORI-approved Project Budget attached hereto as Attachment D; A completed Conflict of Interest Disclosure Form for the Project attached hereto as Attachment E; and Any relevant special terms and conditions applicable to this Project as set forth in any attachments or addendums to this Project Statement.
