Program of Assertive Community Treatment. A multi-disciplinary team approach to providing acute, active, ongoing, and long-term community-based psychiatric treatment, assertive outreach, rehabilitation and support. The program team provides assistance to Enrollees to maximize their recovery, ensure Consumer-directed goal setting, assist individuals in gaining a sense of hope and empowerment, and provide assistance in helping the Enrollees served become better integrated into the community. Services are provided in the community and are available, as needed by the Enrollee, 24 hours a day, seven days a week, 365 days a year. Emergency Services Program (ESP) - services provided through designated contracted ESPs, and which are available seven days per week, 24 hours per day to provide treatment of any individual who is experiencing a mental health crisis. The multidisciplinary PACT team (i.e., psychiatrist, social workers, nurses, and other professionals) provides rehabilitation, support and treatment services to individuals with a primary diagnosis of schizophrenia, bipolar disorder or a major psychotic disorder. Program activities include: • employment counselling; • skill development; • vocational crisis intervention and management; • pre-employment training; • education support and facilitation; • job search development skills; • accessing the hidden job market; and • ongoing active employment support. In 2009/2010, XXXX Xxxxxxxx provided services to 90 individuals, of which 40 were involved in LMAPD eligible activities. Of those, 30 were employed, 6 participated in volunteer work, 10were involved in active job search and 4 attended school. In 2009/2010, PACT Xxxxx provided services to 50 individuals, of which 18 were involved in LMAPD eligible activities. Of those, 7 were employed, 3 participated in volunteer work, 8 were involved in active job search and 2 attended school.

Of those, 28 were involved in work or employment activities, 15 obtained full-time or part-time employment and continued to use PACT services after work was secured in order to maintain their employment and 5 secured full- time competitive employment. During the first year in operation, the PACT Xxxxx location provided services to 25 individuals. One is employed on a casual basis and 1 is volunteering. Proctors Proctors are casual support workers who provide support and assistance to individuals with a mental illness in the areas of living, learning and working. These workers fall under the direction of the Community Mental Health Workers. The portion of the Xxxxxxx Program that is cost-shared under the LMAPD relates to the percentage of time focused on work-related supports such as job coaching, individualized “Choose” activities, and work skills training. In 2007/08, proctors provided services to 800 clients. Of those, 128 clients received employment-related Xxxxxxx services.
