Procedure for Requesting Leaves. Requests for a leave of absence must be submitted in writing by the employee to the employee’s immediate supervisor at least ten (10) working days in advance of the date the leave is to commence, except in emergency situations. The request for the leave of absence shall state the reason for the leave and the exact dates on which the leave is to begin and end. Authorization or denial of a leave of absence shall be furnished to the employee in writing by the Employer. Any request for an extension of a leave of absence must be submitted in writing to the Employer at least ten (10) working days in advance of the expiration date of the original leave, stating the reason for the extension request and the exact revised date the employee is expected to return to work. Authorization or denial of the extension request shall be furnished in writing to the employee by the Employer.

Procedure for Requesting Leaves. A leave of absence, as provided for in this Article, is a written authorized absence from work granted by the Employer. Requests for a leave of absence shall be submitted in writing by the employee to the Department Head at least ten (10) calendar days in advance, except in emergency situations. The request shall state the reason for the leave of absence; the exact date on which the leave begins; and the exact date on which the employee is to return to work. The Employer shall furnish authorization or denial for a leave of absence request to the employee in writing. Additional requirements for specific leaves are included in the following sections dealing with the specific leave. Failure to return to work on the date scheduled shall be cause for termination. Extension beyond the return date designated on the original leave of absence may be granted. The Department Head shall furnish approval or denial in writing to the employee. Written application for an extension must include the reason for the extension; the exact revised date on which the employee is to return to work; must be made at least ten (10) calendar days prior to the expiration date of the original leave of absence (except in those instances where it is not possible to meet the ten (10) calendar day requirement); and must be consistent with any specific eligibility and time limit requirements listed in the following sections dealing with that specific leave. Leaves of absence are to be used for the purpose intended and employees shall make their intent known when applying for such leaves. Employees shall not accept employment elsewhere while on leave of absence, unless agreed to by the Department Head. Acceptance of employment or working for another employer without prior approval while on a leave of absence shall result in immediate termination of employment.

An employee requesting Family and Medical Leave may request to retain up to one-half (½) of their accrued vacation time as of the date the Family and Medical Leave begins. Such request must be made in writing prior to the commencement of the leave unless the employee is prohibited from doing so because of an emergency. Once made, the retention of vacation time cannot be revoked.
