PRIVACY POLICY ACKNOWLEDGEMENT. I acknowledge that I have received a copy of the Privacy Practices and Patient Rights for Gladstone Psychiatry & Wellness, LLC. I understand that this consent includes my agreement that Gladstone Psychiatry & Wellness, LLC can use private health information for my treatment and the billing of my treatment as defined in the Privacy Practices and Patient Rights. Please review the document Notice of Privacy Practices & Patient Rights on pages 3-4 of this document for further details.

PRIVACY POLICY ACKNOWLEDGEMENT. I acknowledge that I have read, understand, and agree to comply with the Providence policies regarding the Health Insurance Portability & Accountability Act of 1996 (“HIPAA”). I also understand that all protected health information(“PHI”) such as patients’ medical records (i.e., admitting/discharge records, labs, imaging), confidential information (i.e., social security number, financial information, card holder data), and other data to which I have knowledge of and/or access to as a result of being a student in the program will be kept confidential.
