I understand that this consent includes my agreement that Gladstone Psychiatry & Wellness, LLC can use private health information for my treatment and the billing of my treatment as defined in the Privacy Practices and Patient Rights. PRIVACY POLICY ACKNOWLEDGEMENT. I acknowledge that I have read, understand, and agree to comply with the Providence policies regarding the Health Insurance Portability & Accountability Act of 1996 (“HIPAA”). I also understand that all protected health information(“PHI”) such as patients’ medical records (i.e., admitting/discharge records, labs, imaging), confidential information (i.e., social security number, financial information, card holder data), and other data to which I have knowledge of and/or access to as a result of being a student in the program will be kept confidential.
