Primary Health Care Services. The PRACTICE shall provide office-based Medical Services to the Members listed above.
Primary Health Care Services. UTMB will subcontract with, or employ, a provider to provide primary care at TJJD Institutions. Each facility will have a designated physician if the primary care provider is not a physician. The facility physician will be a Texas licensed physician and serve as the medical health authority for the Institution. A Provider will be at the Institution at least once per week. In those facilities where a mid-level Provider provides the primary care, a physician will be available to address Youth complaints as needed.
Primary Health Care Services. EFM shall provide office-based Medical Services to the Patient Members listed on page 1 of this Agreement (in accordance with the limitations of Paragraph 3 below).
Primary Health Care Services shall be deemed to include those medical and hospital services and benefits provided by a Primary Care Practitioner, to which Enrollees are entitled.
Primary Health Care Services. The PRACTICE shall provide office-based Medical Services to the Members listed below. Each member is entitled to up to 25 visits annually as part of their membership.
Primary Health Care Services are provided in a comprehensive, integrated, and accessible manner and in a sustained partnership with the student, his or her family, and his or her primary care provider in order to promote health manage, or treat chronic disease with parental consent (unless otherwise allowed by current Georgia law for Confidential Services) and are defined as: • Health Maintenance (well care, immunizations) • Chronic Disease Management • Mental health screening and assessment followed with appropriate services • Acute illness care • Oral health screenings and assessment followed with appropriate services • Non-specialty medical, dental - restorative, mental health, and substance abuse interventions • Referral for other services not available at the health center
Primary Health Care Services. Primary health care services include services that are provided outside the hospital setting by a general practitioner, pharmacist, allied health professionals, community and child health nurses, Home and Community Care and mental health workers. Primary health care services aim to: Keep people well rather than fix them when they are sick Empower the consumer to safeguard their own health through promoting the philosophy of self-responsibility and self-management Deliver culturally sensitive care
Primary Health Care Services. PPMT shall provide, in a prompt and professional manner, Primary Health Care Services, including consultation with health care specialists for the entire inmate population at the MCDF, which is located at 0000 Xxxxxx Xx, Missoula, MT 59808. Said services shall be provided 24 hours per day, seven days per week, with the staffing and service hours contained in the Scope of Services. In addition, PPMT shall:

Primary Health Care Services a. For a minimum of 24 hours per week, VGMHC will provide SBHC primary healthcare to students within the Beaverton School District in accordance with SBHC protocols and certification requirements and the Xxxxxxxx Xxxxxx Memorial Health Center policies and procedures.
