July 2015June 2016: NBMLHD children under 18 years – Letter of Agreement 2013– 2016 Hunter Trial in 3 LGAs: Newcastle, Lake Macquarie & Maitland - MOU EnableNSW role in transition 1 July 2016 – 30 June 2018 Transition to full scheme across NSW – LHD Phasing (Working Arrangement in progress) NDIS Roll Out EnableNSW phasing – Year 1 • EnableNSW consumers linked to FACS clients to transition in Year 1 – 7,557 (33%) • EnableNSW linked to FACS clients who have transitioned – 1,780 • EnableNSW consumers <65 years not linked to FACS clients – 15,193 Transition Funding • If a consumer is under 65 years of age and currently receives Orthotic/FW services through EnableNSW, they may be eligible for the NDIS. • If a consumer is under 65 years of age and currently receives Prosthetic Limb services through EnableNSW, they will be eligible for the NDIS. • If an EnableNSW consumer is eligible and becomes an NDIS Participant, their funding supports will be provided by NDIS, when they have a plan in place. Reasonable and Necessary What is it? • be related to the participant’s disability • not include day-to-day living costs that are not related to a participant’s disability support needs • represent value for money • be likely to be effective and beneficial to the participant, and • take into account informal supports given to participants by families, carers, networks, and the community. How will EnableNSW assist transition? • Once a person is deemed eligible for the NDIS, EnableNSW can provide them with a list of equipment and services they access through EnableNSW to take to their NDIS planning meeting. • If your client is currently using EnableNSW, or has used our services in the past, please continue to send ERFs to us until they are confirmed as an NDIS Participant and have an NDIS Plan in place. Applications are processed within existing guidelines and waiting times. • EnableNSW and the NDIA will be working together to ensure funding is covered for any equipment requests in progress. • EnableNSW is a registered provider and clients can still utilise EnableNSW for supply and coordination of supports after they have transitioned.   • Beta Services From time to time, We may invite You to try Beta Services at no charge. You may accept or decline any such trial in Your sole discretion. Beta Services will be clearly designated as beta, pilot, limited release, developer preview, non-production, evaluation or by a description of similar import. Beta Services are for evaluation purposes and not for production use, are not considered “Services” under this Agreement, are not supported, and may be subject to additional terms. Unless otherwise stated, any Beta Services trial period will expire upon the earlier of one year from the trial start date or the date that a version of the Beta Services becomes generally available. We may discontinue Beta Services at any time in Our sole discretion and may never make them generally available.   • Prosthodontics We Cover prosthodontic services as follows: • Removable complete or partial dentures, for Members 15 years of age and above, including six (6) months follow-up care; • Additional services including insertion of identification slips, repairs, relines and rebases and treatment of cleft palate; and • Interim prosthesis for Members five (5) to 15 years of age. We do not Cover implants or implant related services. Fixed bridges are not Covered unless they are required: • For replacement of a single upper anterior (central/lateral incisor or cuspid) in a patient with an otherwise full complement of natural, functional and/or restored teeth; • For cleft palate stabilization; or • Due to the presence of any neurologic or physiologic condition that would preclude the placement of a removable prosthesis, as demonstrated by medical documentation.

  • PRODUCER Provide the producer’s name, address (including country), e-mail address, and telephone number, if different from the certifier or exporter or, if there are multiple producers, state “Various” or provide a list of producers. A person who wishes for this information to remain confidential may state “Available upon request by the importing authorities”. The address of a producer shall be the place of production of the good in a Party’s territory.
