The Physical Therapy Services clause defines the scope and terms under which physical therapy will be provided to a patient or client. Physical Therapy Services. HLC may provide physical therapy services, if required in Student’s IEP or desired by School District. Such services are provided at an additional cost of One Hundred Fifty Dollars ($150.00) per hour. If required by Student’s IEP, occupational therapy services will be provided for the number of hours required in the IEP. By initialing on this line, School District accepts the Physical Therapy Services and agrees to the terms or paragraph 5(d) above .
Physical Therapy Services. Contractor shall provide proof of State of Ohio Licensure, and resume, to the Board for all relevant provider employees and kept posted in the designated office area at the Board’s facility.
Physical Therapy Services. The EMPLOYER will provide ARTISTS’ with the services of a Certified Athletic Trainer or Physical Therapist.
a. Physical Therapy services will be available at the trainer’s or Physical Therapist’s regular work site for not less than one (1) Artist Free Day per work week and at the Kansas City Ballet studios for not less than two (2) Artist work days per work week.
b. In addition, when the ARTISTS are scheduled to participate in a Dress Rehearsal, Performance, or school show performance an Athletic Trainer or Physical Therapist shall be available at the site for at least one (1) hour before, during, and at the request of any ARTIST, shall remain after the conclusion of such Artistic Activity for up to a one half (½) hour. However, with the exception of a two (2) Performance day when the trainer or Physical Therapist will be available for both shows, the Employer will not be obligated to provide Physical Therapy Services twice in one (1) day, provided that services are provided on one (1) Free Day and two (2) work days per work week.
Physical Therapy Services. Kindred shall provide such physical therapists and/or physical therapy assistants and/or physical therapy aides as shall be required to provide physical therapy services to the Health Care Facility which may include, without limitation, the following:

During the term of this Agreement, School shall provide access to designated space to Contractor at School’s ▇▇▇▇▇▇▇▇▇ - ▇▇▇▇▇▇ High School location in which to provide physical therapy services (the “Physical Therapy Services”) set forth in detail on Exhibit E attached hereto to students, student athletes, employees, and faculty. Hereinafter, Physical Therapy Services and Athletic Training Services may be collectively referred to as “Services.”

Physical Therapy Services. Physical therapy includes services to address the promotion of sensorimotor function through enhancement of musculoskeletal status, neurobehavioral organization, perceptual and motor development, cardiopulmonary status, and effective environmental adaptation. These services include— (i) Screening and assessment of infants and toddlers to identify movement dysfunction; (ii) Obtaining, interpreting, and integrating information appropriate to program planning to prevent, alleviate, or compensate for movement dysfunction and related functional problems; and (iii) Providing individual and group services or treatment to prevent, alleviate, or compensate for movement dysfunction and related functional problems.
Physical Therapy Services. The Employer will provide Artists with the services of a Certified Athletic Trainer or Physical Therapist which will be available at the Employer's studios for not less than one (1) workday per week. Physical Therapy Services shall also be provided at the place of performance as needed.
