I hereby agree to allow the Spooky Nook Sports to record and publish photos and videos (including audio) of myself for the purpose of promoting Spooky Nook Sports in a manner that does not violate NCAA Bylaw 12.5.2 and for documenting and/or reporting events and activities. I understand photographs, video and/or audio tape recordings to be taken of myself and/or family members at practice, during competition, recreational play, as well as other Spooky Nook Sports related events. I understand that this media will be produced and used for promotional purposes that do not violate NCAA Bylaw 12.5.2, and I authorize Spooky Nook Sports to use my/our photograph, video and/or audio recording on its Website and social media platforms, such as Facebook, Twitter, YouTube, FourSquare and Pinterest, etc., as well as other official printed publications without further consideration. In addition, I acknowledge Spooky Nook Sports’ right to crop or treat the media at its discretion, and I also acknowledge that Spooky Nook Sports may choose not to use my/our image at this time, but may do so as its own discretion at a later date. I also understand that once I, or my family members, image(s) have been captured, they may be posted on the Spooky Nook Sports Website or social media platforms, the image can be downloaded by any computer user on or off the premises of the Sports Complex. PHOTOGRAPHY RELEASE. The Licensee grants to University permission to: record Licensee’s appearance on videotape, audiotape, film, photograph or any other medium, whether now known or hereafter conceived or created (collectively, “Recordings”); use Licensee’s name, likeness, voice and biographical material in connection with such Recordings; exhibit or distribute such Recordings in whole or in part without restrictions or limitations for any legal purpose; including without limitation educational or promotional purposes, which the University and those acting pursuant to its authority deem appropriate; and copyright such Recordings in its own name or to publish, to market and to license or assign rights in such Recordings without consideration, compensation, accounting or report to Licensee.
PHOTOGRAPHY RELEASE. Participant and his/her parent(s) or legal guardian(s) hereby grant to the Host, its representatives, and employees the right to take photographs of Participant in connection with Participant’s participation in the Activities. Participant hereby authorizes the Host to copyright, use, and publish the same in print and/or electronically. Participant hereby agrees that the Host may use such photographs of Participant for any lawful purpose, including but not limited to publicity, illustration, advertising, and Web content.
PHOTOGRAPHY RELEASE. The BFRC periodically photographs during BFRC sponsored programs. These pictures may be used for advertising, Internet (website/Facebook) and around the facility. Occasionally pictures may be taken and submitted to the newspaper or other media as well. By signing the team roster/liability waiver on the next page you are also giving the BFRC/City of Belle Fourche permission to use these photographs.

I hereby fully and forever discharge and release Rugged Races LLC and its agents from any claim for damages of any kind (including, but not limited to, invasion of privacy; defamation; false light or misappropriation of name, likeness or image) arising out of the use or publication of photographs of me and/or those of my minor children (if applicable) by Rugged Races LLC or its agents, and covenant and agree not to xxx or otherwise initiate legal proceedings against Rugged Races LLC or its agents for such use or publication on my own behalf or on behalf of my minor children. All grants of permission and consent, and all covenants, agreements and understandings contained herein are irrevocable.

PHOTOGRAPHY RELEASE. The Lessee authorizes YDBG to use photographs taken and released by the event photographer or other individuals (guests, YDBG employees, vendors, etc.) on the date of the event for purposes related to the promotion of YDBG event rental, included but not limited to; print advertising, television advertising, promotional internet websites and social media networks. The Lessee hereby releases YDBG and its legal representatives and assigns from all claims and liability relating to said photographs.

Unless a negative answer was provided on section B of the enrollment form, the Parent gives Kishami Academy permission to use in any means required any images of the Student and/or family members involved with the School’s program or functions for purposes of education, advertisement, promotion, or communication.

Photographs or other graphic, sound, or other image, likeness, recording, etc., may be made of my dog(s) by Dogtopia of and that such may be used for any purpose without compensation, and I release to Dogtopia of and Dogtopia Enterprises, LLC and its affiliates and subsidiaries all rights that I may possess or claim to such image, likeness, recording, etc. FEES & PAYMENTS. Payment is required when services are rendered. If any amounts remain due after 30 days, Dogtopia of reserves the right to impose interest at the rate of 1.5% per month until paid. If Dogtopia of pursues collection proceedings, I will pay reasonable attorney’s fees and costs of collection. HOURS OF OPERATIONS & LATE PICK-UP FEES. Please refer to our brochure or website for store hours. We reserve the right to impose a late fee for dogs picked up after regular business hours. If your dog is not picked-up within 30 minutes of closing, we will assume that you are boarding and will impose an overnight boarding charge.
PHOTOGRAPHY RELEASE. I hereby consent and authorize Old Trail Swim Club (and their respective owners, directors, partners, officers, operators, employees and other agents) to copyright, use and publish any images in any format taken of me, and/or my family and/or guest(s) while utilizing club facilities or attending club events. I understand these images may be used for a variety of purposes and may appear on the Club website, in the Club newsletter, for promotional materials or any other media now known or to be developed. I also understand that Old Trail Swim Club or any entity authorized by Old Trail Swim Club will use the images exclusively for Club-related purposes and not for any other commercial gain. Because anyone can download an image from the Internet or make copies from printed materials, I also agree that Old Trail Swim Club is not responsible for unauthorized use of the images, and I am aware that I am not entitled to any compensation and that the images may appear with or without my name.
