Common use of Paternity Leave Clause in Contracts

Paternity Leave. An employee will provide to the employer at least ten weeks prior to each proposed period of paternity leave, with: 33.4.1 a certificate from a registered medial practitioner which names his spouse, states that she is pregnant and the expected dated of confinement, or states the date on which the birth took place; and 33.4.2 written notification of the dates on which he proposes to start and finish the period of paternity leave; and 33.4.3 a statutory declaration stating:

Paternity Leave. 29.4.1 An employee will provide to the employer at least ten weeks prior to each proposed period of paternity leave, with: 33.4.1 29.4.1 (a) a certificate from a registered medial medical practitioner which names his spouse, states that she is pregnant and the expected dated of confinement, or states the date on which the birth took place; and 33.4.2 29.4.1 (b) written notification of the dates on which he proposes to start and finish the period of paternity leave; and 33.4.3 a statutory declaration stating:

Paternity Leave. Paternity Leave. 37.4.1 An employee will provide to the employer at least ten weeks prior to each proposed period of paternity leave, with: 33.4.1 37.4.1 (a) a certificate from a registered medial medical practitioner which names his spouse, states that she is pregnant and the expected dated date of confinement, or states the date on which the birth took place; and 33.4.2 37.4.1 (b) written notification of the dates on which he proposes to start and finish the period of paternity leave; and 33.4.3 a statutory declaration stating:
