Common use of Paternity Leave Clause in Contracts

Paternity Leave. An employee will provide to the employer, at least ten weeks prior to each proposed period of paternity leave : (a) a certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date of confinement, or states the date on which the birth took place; and (b) written notification of the date on which he proposes to start and finish the period of paternity leave.

Paternity Leave. (a) An employee Employee will provide to the employer, Employer at least ten weeks prior to each proposed period of paternity leave leave, with: (ai) a certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date of confinement, or states the date on which the birth took place; and (bii) written notification of the date dates on which he proposes to start and finish the period of paternity leave.; and

Paternity Leave. 7.4.5.1 An employee will provide to the employer, employer at least ten weeks prior to each proposed period of paternity leave leave, with: 7.4.5.1 (a) a certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date dates of confinement, or states the date on which the birth took place; and 7.4.5.1 (b) written notification of the date dates on which he proposes to start and finish the period of paternity leave.; and

Paternity Leave. An employee will provide to the employer, employer at least ten weeks prior to each proposed period of paternity leave leave, with: (a) 33.4.1 a certificate from a registered medical medial practitioner which names his spouse, states that she is pregnant and the expected date dated of confinement, or states the date on which the birth took place; and (b) 33.4.2 written notification of the date dates on which he proposes to start and finish the period of paternity leave.; and 33.4.3 a statutory declaration stating:

Paternity Leave. 25.6.1 An employee will provide to the employer, employer at least ten weeks prior to each proposed period of paternity leave with: (a) a certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date of confinement, or states the date on which the birth took place; and 25.6.1 (b) written notification of the date dates on which he proposes to start and finish the period of paternity leave.; and

Paternity Leave. An employee will provide to the employer, employer at least ten weeks prior to each proposed period of paternity leave leave, with: (a) 28.4.1. a certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date dated of confinement, or states the date on which the birth took place; and (b) 28.4.2. written notification of the date dates on which he proposes to start and finish the period of paternity leave.; and 28.4.3. a statutory declaration stating:

Paternity Leave. An employee will provide to the employer, employer at least ten 10 weeks prior to each proposed period of paternity leave leave, with: (a) a 27.8.1 A certificate from a registered medical practitioner which names his the employee’s spouse, states that she is pregnant and the expected date of confinement, or states the date on which the birth took place; and (b) written 27.8.2 Written notification of the date dates on which he the employee proposes to start and finish the period of paternity leave.; and

Paternity Leave. 46.4.1 An employee will provide to the employer, CWW at least ten weeks prior to each proposed period of paternity leave leave, with: (a46.4.1(a) a certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date of confinement, or states the date on which the birth took place; and 46.4.1 (b) written notification of the date dates on which he proposes to start and finish the period of paternity leave.; and

Paternity Leave. 25.5.1 An employee will provide to the employer, employer at least ten weeks notice prior to each proposed period of paternity leave leave, with: 23.5.1 (a) a certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date of confinement, or states the date on which the birth took place; and 23.5.1 (b) written notification of the date dates on which he proposes to start and finish the period of paternity leave.; and

Paternity Leave. 29.5.1. An employee will provide to the employer, employer at least ten weeks prior to each proposed period of paternity leave leave, with: 29.5.1 (a) a certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date of confinement, or states the date on which the birth took place; and 29.5.1 (b) written notification of the date dates on which he proposes to start and finish the period of paternity leave.; and

Paternity Leave. An employee employee, will provide to the employer, Corporation at least ten weeks prior to each proposed period of paternity leave :leave:- (a) a certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date of confinement, or states the date on which the birth took place; and (b) written notification of the date on which he proposes to start and finish the period of paternity leave.

Paternity Leave. 15.9.1 An employee will provide to the employer, employer at least ten 10 weeks prior to each proposed period of paternity leave leave, with: 15.9.1 (a) a certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date of confinement, or states the date on which the birth took place; and 15.9.1 (b) formal written notification of the date dates on which he proposes to start and finish the period of paternity leave.; and

Paternity Leave. 29.6.1 An employee will provide to the employer, employer at least ten (10) weeks prior to each proposed period of paternity leave leave, with: (a) a A certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date of confinementbirth, or states the date on which the birth took place; and 29.6.1 (b) written notification of the date dates on which he proposes to start and finish the period of paternity leave.; and

Paternity Leave. 33.4.1 An employee will provide to the employer, Employer at least ten weeks prior to each proposed period of paternity leave leave, with: 33.4.1 (a) a certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date of confinement, or states the date on which the birth took place; and 33.4.1 (b) written b)written notification of the date dates on which he proposes to start and finish the period of paternity leave.; and

Paternity Leave. An (i) A male employee will provide to the employer, employer at least ten 10 weeks prior to each proposed period of paternity leave leave: (a1) that a certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date of confinement, or states the date on which the birth took place; , and (b2) written notification of the date proposed dates on which he proposes to start and finish the period of paternity leave.leave will start and finish and (3) a statutory declaration stating:

Paternity Leave. 37.4.1 An employee will provide to the employer, employer at least ten weeks prior to each proposed period of paternity leave leave, with: 37.4.1 (a) a certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date of confinement, or states the date on which the birth took place; and (b) written notification of the date on which he proposes to start and finish the period of paternity leave.

Paternity Leave. An employee will provide to the employer, Employer at least ten 10 weeks prior to each proposed period of paternity leave : (a) leave, with : • a medical certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date of confinement, or states the date on which the birth took place; and (b) and • written notification of the date dates on which he proposes to start and finish the period of paternity leave.; and • a statutory declaration stating :

Paternity Leave. 20.9.1 An employee will provide to the employer, employer at least ten weeks prior to each proposed period of paternity leave leave, with: (a) a certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date of confinement, or states the date on which the birth took place; and 20.9.1 (b) written notification of the date dates on which he proposes to start and finish the period of paternity leave.; and

Paternity Leave. An employee will provide to the employer, employer at least ten 10 weeks prior to each proposed period of paternity leave with: (a) 47.4.1 a certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date of confinement, or states the date on which the birth took place; and (b) 47.4.2 written notification of the date dates on which he proposes to start and finish the period of paternity leave.; and 47.4.3 a statutory declaration stating: Municipal Council of Roxby Downs Enterprise Agreement 2010

A certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date of confinement, or states the date on which the birth took place; and (b) written notification of the date on which he proposes to start and finish the period of paternity leave.

Paternity Leave. 28.4.1 An employee will provide to the employer, employer at least ten weeks prior to each proposed period of paternity leave leave, with: 28.4.1 (a) a certificate from a registered medical practitioner officer which names his spouse, states that she is pregnant and the expected date of confinement, or states the date on which the birth took place; and 28.4.1 (b) written notification of the date dates on which he proposes to start and finish the period of paternity leave.; and

Paternity Leave. 7.2.4.1 An employee will provide to the employer, employer at least ten 10 weeks prior to each proposed period of paternity leave leave, with: 7.2.4.1 (a) a certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date of confinement, or states the date on which the birth took place; and 7.2.4.1 (b) written notification of the date dates on which he proposes to start and finish the period of paternity leave.; and

Paternity Leave. 39.6.1 An employee will provide to the employer, employer at least ten weeks prior to each proposed period of paternity leave leave, with: 39.6.1 (a) a certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date of confinement, or states the date on which the birth took place; and 39.6.1 (b) written notification of the date dates on which he proposes to start and finish the period of paternity leave.; and

Paternity Leave. 38.4.1 An employee will provide to the employer, Company at least ten weeks prior to each proposed period of paternity leave leave, with: 38.4.1 (a) a certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date of confinement, or states the date on which the birth took place; and 38.4.1 (b) written notification of the date dates on which he proposes to start and finish the period of paternity leave.; and

Paternity Leave. 17.6.1 An employee will provide to the employer, employer at least ten weeks prior to each proposed period of paternity leave leave, with: (a) a certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date of confinement, or states the date on which the birth took place; and (b) written notification of the date on which he proposes to start and finish the period of paternity leave.

Paternity Leave. 20.4.1. An employee will provide to the employer, employer at least ten weeks prior to each proposed period of paternity leave leave, with: (a) 20.4.1.1. a certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date of confinement, or states the date on which the birth took place; and (b) 20.4.1.2. written notification of the date dates on which he he/she proposes to start and finish the period of paternity leave.; and

Paternity Leave. (a) An employee will provide to the employer, Company at least ten weeks notice prior to each proposed period of paternity leave leave, with: (ai) a certificate from a registered medical practitioner which names his spouse, states that she is pregnant and the expected date of confinementbirth, or states the date on which the birth took place; and (bii) written notification of the date dates on which he proposes to start and finish the period of paternity leave. (b) The employee may take one week’s paid paternity leave at the time of the birth of the child.
