Not Insurance. You understand that the Practice is not health insurance, a health benefit plan or a health insurance provider. The Practice is not responsible for making any health care available to you other than the Services. The Practice is not intended as and is not a substitute for, emergency medical services. In the event of a medical emergency, you should call 911. Practice encourages You to maintain adequate Insurance coverage at all times.
Not Insurance. A direct primary care service agreement is not an insurance policy and is not subject to regulation by the Department of Professional and Financial Regulation, Bureau of Insurance. [PL 2017, c. 112, §1 (NEW).]
Not Insurance. We are not an insurance company and do not recommend or endorse any insurance company. You are responsible for your Vehicle insurance. The Services do not provide your Vehicle insurance and are not an insurance product. The Services are provided as a convenience. The payments You make for any of the Services are not related to the value of your Vehicle or any property in it, or the cost of any injury to or damages incurred by You. You should obtain and maintain appropriate insurance for your Vehicle and for any related risks.

It will not cover hospital services or any services not personally provided by FCIM. You acknowledge that FCIM has advised You to obtain or keep in full force your health insurance policy(ies) or plans that will cover You for general healthcare costs. You acknowledge that this Agreement is not a contract that provides health insurance, and this Agreement is not intended to replace any existing or future health insurance or health plan coverage that You may carry.

ANY INSURED PRODUCTS MADE AVAILABLE THROUGH THE BENALIGN WEBSITE WILL BE PROMINENTLY IDENTIFIED AS INSURED SERVICES FOR CLARIFICATION AND COMPLIANCE PURPOSES. ALL INSURED PRODUCTS MADE AVAILABLE ARE QUOTED AND SUPPORTED THROUGH A THIRD-PARTY LICENSED INSURANCE AGENCY.
