Neuter Agreement. Permanent ownership of this dog is contingent upon your compliance with this Addendum. Proof of the spay/neuter must be received by the (PURCHASER’S NAME) within fourteen (14) days after the surgery. In the event that the dog’s health does not allow this agreement to be honored. Xxxx’s Havanese must be provided with a statement from your veterinarian that this dog is not yet in physical or emotional condition for surgery. Based upon the veterinarian’s evaluation of this dog the (purchaser) will contact the veterinarian and establish the earliest data that the procedure can be performed. (PURCHASER) will notify the BREEDER of this amendment in writing, at which time all other conditions of this agreement will apply and be enforced. This is the responsibility of the PURCHASER, not the veterinarian, to ensure that Xxxx’s Havanese has received veridcation that the surgery has been performed. Failure to comply with this agreement by the data bellow, unless otherwise agreed to in writing, will be considered a breach of the Contract, and PURCHASER will transport this puppy back to Luda’s Havanese and will not be entitled to a refund. By my signature bellow, I agree to have this dog spayed/neutered by not later than , and I understand that this spay/neuter agreement is an agreement that this dog xxxx not produce a litter of puppies either as purposely bred or by accidental breeding. Neuter Agreement. All dogs made available by Barking Mad Animal Rescue will be spayed or neutered before placing for adoption unless special circumstances are in effect. Special Circumstances must be verified by a veterinarian. No exceptions.
Neuter Agreement. If puppy is purchased as a pet Buyer agrees to neuter or spay the puppy by no later than puppy’s sixth month. Buyer to provide Show Tyme Shih Tzu with a copy of the certificate of Spay / Neuter from a certified vet before the puppy reaches 7 months of age. If Buyer fails to Spay/Neuter this puppy and fails to provide proof to Show Tyme Shih Tzu by the time the puppy is 7 months of age, rights and ownership of the puppy will automatically be terminated and puppy will go back to Show Tyme Shih Tzu. Show Tyme may immediately assume possession of the puppy with no refund to the buyer. Buyer will be responsible for any and all fees incurred (attorney, court costs, filing, travel, shipping cost for puppy, etc.) in the safe recovery of returning the puppy to Show Tyme Shih Tzu. Registration papers- AKC limited registration papers, if desired & offered as part of your purchase will be released AFTER proof of spay or neuter is received. Buyer must request papers in conjunction with delivery of paperwork above within the 7 month timeframe. Puppies sold on limited registration may not be bred or shown in AKC conformation shows. Certified Pet Registration papers may be given immediately when you receive your puppy at Buyer request. Buyer agrees that said dog will live inside the Buyer’s home and will not be housed outside or in a garage type setting for any length of time. Buyer agrees said dog will be socialized and treated as one of the family. (Buyer is requested to send a family photo of puppy with family members within 60 days of leaving Breeder’s home, and ongoing photos and updates are greatly appreciated!) Once said dog has left the home of Show Tyme Shih Tzu, Show Tyme Shih Tzu is not responsible for any infection, allergies, illness or behavioral problems that may occur. This guarantee is against life threatening congenital defects limited to heart, liver, kidney, and lungs. This guarantee is NOT transferable; applies to original Buyer only and does not transfer if puppy is resold or given away to live anywhere other than Buyer’s full time Residence. The buyer agrees that the puppy will never be placed in a shelter, rescue, or sold to a broker, per shop, or puppy mill. If the buyer is unable to keep the puppy, the buyer is required to contact Show Tyme Shih Tzu before ever placing the puppy. Neuter Agreement. This neuter agreement is an agreement that the male Pet, will not be purposely bred or by accidental breeding. The rescuer’s male Pet adopted as a puppy/kitten will be neutered by 6 months of age. If over 6 months of age the Pet will be neutered before placement unless the Pets heath does not permit Pet to be neutered at this time. If the Pet’s heath does not allow this agreement to be honored, permanent ownership by the adopter is contingent upon a veterinary statement. This statement will be by the veterinarian that has seen this Pet, examined this Pet, and has evaluated this Pet as not yet in emotional and/or physical condition for neutering. Based on this situation, Rescuer, not the adopter, and veterinarian will extend the length of time to be neutered. Immediately after the Pet’s condition has improved said Pet will be neutered. Verification of sterilization by veterinarian is to be mailed to rescuer within fourteen (14) days of the neutering. It is the responsibility of the adopter, not the veterinarian to be sure that the rescuer has received this statement. If this agreement is not honored the adopter will acknowledge and agree that the contract has been violated and the adopter will transport the Pet back to rescuer with no refund. Collars and Tags The Pet will not wear a pinch (cleat) collar or any similar article around the neck for the purpose of any basic training, any basic correcting, or for permanent use of identification. The pinch (cleat) collar are only to be used to correct a behavior problem which is an isolated incident and only under guidance of a professional Pet trainer, then not to be used for any other reason. The Pet shall at all times be required to were a nylon belt buckle collar or harness with the rabies tag, identification tag displaying the home phone number and the county license. The chain collar (often referenced as a choke chain or training chain) will only be used to walk the Pet on a leash. A leash is required when the Pet is taken out in a public area which enforces a leash law, any area which requires total control over the Pet at all times, and any area which requires the Pet to be leashed at all times when walked in a residential area.

SCGSR will then reidentify the guidelines based on the Dog’s condition. It is the responsibility of the Adopter, not the veterinarian, to be sure that SCGSR has received this statement. If these provisions are not honored the Adopter will acknowledge and agree that this Adoption Contract has been violated and the Adopter will transport the Dog back to SCGSR with no refund.
