Membership Category. There are four classes of membership in the Foundation: Platinum Membership, Gold Membership, Silver Membership and At-Large Membership. The membership class applicable to the Member (the “Membership Category”) is indicated on the first page of this Agreement. The rights and obligations of members in the Membership Category are set forth in Foundation’s Bylaws and in the applicable Membership Policies (the “Member Policies”).

Membership Category. This Agreement describes the potential opportunities available to the Health Organization as a WOHT Health Organization Member and the relationship between the two parties. As a WOHT Member there is the expectation that to the extent that resources allow, the Health Organization, and Health Organization’s Staff will participate in and support the work of the WOHT and will collaborate with other WOHT Members to collectively advance the WOHT’s mission and goals, fulfill the expectations established by the Ministry of Health, and evolve to its mature state as an OHT.‌

  • Membership Dues Association membership dues, as explicitly approved by the Trustees;

  • Membership Benefits The benefits of CamCare programs are available only while your membership is current and active. Benefits are term year specific so they can only be used during the current active plan agreement period.   • Annual Membership Fee You agree to pay in advance a nonrefundable annual membership fee shown on page 1, whether or not you use your Card or Credit Card account.
