Medical Practitioners. B.1.4.1 This agreement shall apply to Employees of the University of Canterbury who are members of the TEU and whose duties include the practice of medicine (as defined from time to time by the New Zealand Medical Council) at the Health Centre and who are required, as a condition of employment, to hold a current practicing certificate.

Medical Practitioners. E.2.3.1 The initial placement of each Doctor on the salary Scale shall be the responsibility of the Director of the Centre, taking into account relevant qualifications and experience.

It is important to list all drugs the student uses regularly. Some foreign schools do not allow students to take drugs, including asthma inhalers, onto school property without approval from a physician and a note from parents. Immunisation Record Vaccinations are essential for enrolment into host schools. The requirements differ from what is advisable for holiday travel and often, from the Australian schedule for immunisations. The following vaccinations are required: • Diphtheria, Tetanus and Pertussis (DTaP): Four (4) or more doses are required, one dose must have been received after the student’s fourth birthday. • Diphtheria, Tetanus and Pertussis booster (Tdap): A Tdap booster is required and must have been administered within 10 years of the end date of the WEP program or, for the USA, received within the past five (5) years. • Polio: Three or more doses (four (4) doses for the USA) are required. One dose must have been administered after the student’s fourth birthday. • Hepatitis B: Countries, except the USA, accept either a two dose or three dose schedule. For the USA, students must have received three (3) doses of Hep B vaccine. Hep B is MANDATORY for applicants to Italy, NOT REQUIRED for applicants to other European countries. • Measles, Mumps, Rubella (MMR): Two (2) doses are required, the first dose after the student’s first birthday and the second dose 28 or more days later OR physician’s verification of disease/s (e.g. date of disease/s or results of antibody test).

  • Nurse Practitioner (Employer Appointed Position) “Nurse Practitioner” means a Registered Nurse appointed as such to a position approved by the employer and who is authorised by the Board under Section 95 of Health Practitioner Regulation National Law (NSW) No 86A. A Nurse Practitioner will have at least three years full-time equivalent experience in an advanced practice role and meets the national competency standards for Nurse Practitioners. A Nurse Practitioner functions autonomously and operates at a level of nursing that uses extended and expanded skills, experience and knowledge assessment, planning, implementation, diagnosis and evaluation of nursing care. A.5.1

  • Medical Insurance The Company shall provide to Executive, Executive's spouse and children, at its sole cost, such health, dental and optical insurance as the Company may from time to time make available to its other executive employees.

  • Medical Examinations An employee may be required by the Employer, at the request of and at the expense of the Employer, to take a medical examination by a physician of the employee's choice. Employees may be required to take skin tests, x-ray examination, vaccination, inoculation and other immunization (with the exception of a rubella vaccination when the employee is of the opinion that a pregnancy is possible), unless the employee's physician has advised in writing that such a procedure may have an adverse affect on the employee's health.

  • Medical Verification The Town may require medical verification of an employee’s absence if the Town perceives the employee is abusing sick leave or has used an excessive amount of sick leave. The Town may require medical verification of an employee’s absence to verify that the employee is able to return to work with or without restrictions.

  • Medical Plan ‌ Eligible employees and dependants shall be covered by the British Columbia Medical Services Plan or carrier approved by the British Columbia Medical Services Commission. The Employer shall pay one hundred percent (100%) of the premium. An eligible employee who wishes to have coverage for other than dependants may do so provided the Medical Plan is agreeable and the extra premium is paid by the employee through payroll deduction. Membership shall be a condition of employment for eligible employees who shall be enrolled for coverage following the completion of three (3) months’ employment or upon the initial date of employment for those employees with portable service as outlined in Article 14.12.

  • Medical Examination Where the Employer requires an employee to submit to a medical examination or medical interview, it shall be at the Employer's expense and on the Employer's time.

  • Medical Services We do not Cover medical services or dental services that are medical in nature, including any Hospital charges or prescription drug charges.

  • Health Care Operations Health Care Operations shall have the meaning set out in its definition at 45 C.F.R. § 164.501, as such provision is currently drafted and as it is subsequently updated, amended or revised.

  • Medical Plans The Employer will maintain the current health (including vision) and dental insurance programs and practices. The Employer shall contribute 80% of the premium charge for PPO plans, 83% of premium for the POS plan, 85% of premium for the HMO plan, 80% for the prescription drug plan and 50% for the dental plan. There shall be no change in the State’s premium subsidy for health benefits plans in Fiscal Year 2012.

  • Medical and Dental Insurance The Company shall pay Employee’s monthly Medical and Dental Insurance premiums in association with Company provided health insurance plans.
