Common use of Medical Notes Clause in Contracts

Medical Notes. .1 The company has the right to ask for a medical note as evidence to substantiate the absence. .2 When a medical note is required is must contain the following: .2.1 the name of the physician or medical practitioner who is providing the note; .2.2 the date on which the employee was seen; .2.3 the date that the physician or medical practitioner estimates that the employee will be able to return to work; .2.4 a clear indication of the employee’s functional abilities and medical restrictions; and
