Medical, Dental & Vision Benefits. A. Effective March 1, 2014, the NECA Family Medical Plan # 16 (hereinafter referred to as the “NECA Health Plan”) will be the only negotiated Plan for all Medical (including Prescription Drug), Dental and Vision (hereinafter collectively referred to as “Health”) Benefits offered to employees in the Northwest, Blythe/Xxxxxx and Gardnerville bargaining units.

Medical, Dental & Vision Benefits. The City shall make available group medical, prescription drug, wellness, dental, and vision benefits to all members and dependents who meet the eligibility requirements of the plan.
Medical, Dental & Vision Benefits. This article applies to the calendar year 2020 only. The City will make available group medical, prescription drug, dental, and vision benefits to all employees and dependents who meet the eligibility requirements of the plan. The City will provide health insurance coverage to Employees through a high deductible healthcare plan with an associated health savings account (HSA) or a Health Reimbursement Account (HRA). An HRA is for those employees ineligible for an HSA based on being covered by other healthcare coverage like Medicare or TRICARE.

Medical, Dental & Vision Benefits. The COBRA provision of the federal law allows you to remain in the Company’s group plan for medical, dental, and vision benefits for up to eighteen (18) months with coverage retroactive to your Separation Date. After your Separation Date, you will receive information about the COBRA continuation of coverage option, including the necessary election forms, at your home address. You must complete and return the election forms within sixty (60) days of your Separation Date to elect COBRA benefits. COBRA premiums are generally 102% of the full benefit cost (employee and employer portions). In an effort to offset this expense and to enhance this benefit, the Company will extend the eighteen (18) months of continued coverage to twenty-four (24) months and make a one-time payment to you equal to the difference of the full COBRA premiums and the amount you would pay for these benefits as an active employee. This payment will be made at the time of your lump sum severance payment and will be taxed accordingly. This payment will be calculated as follows: (Full COBRA premium – current active employee premiums) x 24 months = one-time payment
Medical, Dental & Vision Benefits. If the Chief Executive Officer elects COBRA Continuation Coverage, he shall continue to participate in all medical, dental and vision insurance plans he was participating in on his Termination Date. If, however, any such plan does not permit his continued participation following the end of the COBRA Continuation Period (as defined below), then the Company will reimburse the Chief Executive Officer for the actual cost to the Chief Executive Officer of any individual health insurance policy obtained by the Chief Executive Officer. To the extent such benefits are available under the above-referenced plans and the Chief Executive Officer had coverage immediately prior to the Separation From Service, such continuation of benefits for the Chief Executive Officer shall also cover the Executive’s dependents for so long as the Chief Executive Officer is receiving benefits under this subsection (a). The provisions of Section 4.2.2 and 4.2.4 of the Plan shall also apply. “COBRA Continuation Period” means the continuation period for medical, dental and vision insurance to be provided under the terms of the Plan and herein which shall commence on the first day of the calendar month following the month in which the Termination Date falls and generally shall continue for an 18-month period.

Medical, Dental & Vision Benefits. The City shall make available group medical, prescription drug, dental, and vision benefits to all employees and dependents who meet the eligibility requirements of the plan. The City will provide health insurance coverage to Employees through a high deductible, health savings account (HSA) or a Health Reimbursement Account (HRA) for those employees ineligible for an HSA based on either Medicare or TriCare enrollment. Annual deductibles shall be $2,500 for single coverage in network and $5,000 for family coverage in network for both medical and pharmacy coverage. In addition, the City will make annual contributions to each participating employees’ health savings account based on the coverage level (single or family) and participation in the City’s Wellness program, Healthy by Choice (HBC) Plus. The benefit plan provides participation- based and results-based wellness incentives to help offset the employee deductible amounts under this plan design. Details of the Healthy by Choice program and contributions are included in Attachment A and incorporated into this Article by reference. For the calendar years beginning January 1, 2014, January 1, 2015, and 2016: Participation - Based Single Coverage: $1,125 Family Coverage: $2,250 Results – Based $150 per health factor for blood pressure, cholesterol and tobacco-free status $ 300 per health factor for weight Participating employees, newly hired during the year shall have the City’s contribution to their HSA or HRA based on the enrollment period for the HBC program in that initial year.

Under COBRA, Employee may continue coverage for a total of 18 months after the Separation Date. To continue Novelis Medical, Dental and Vision coverage for Employee and eligible family members, Employee must complete and return the COBRA election form to BSL and otherwise fulfill all COBRA requirements.
