Medical Consultation. I agree to consult with my personal physician about necessary immunizations and any other medical matters relating to my participation in an international program. I understand that I am strongly encouraged to disclose to the Study Abroad Office and/or Program Leader(s) any personal, physical, medical, and/or psychological or emotional concerns/issues I might have which would, without reasonable accommodation, prevent me from performing my responsibilities on my program. I understand that if I have a disability which requires reasonable accommodation by the program provider, I will advise the Program Leader(s) at a time sufficiently prior to the commencement of the program to allow the Program Leader(s)/providers to make reasonable accommodations.
Medical Consultation. The Athletic Trainer will assist the athlete in obtaining timely medical care. The Athletic Trainer will follow School policy for injury care and reporting of injuries, will notify the athlete’s parents of the need of medical consultation, offer to contact the family physician, and, if necessary, put the parents in contact with an orthopaedic surgeon or sports medicine specialist.
Medical Consultation. A pregnant employee who is advised by her physician to commence pregnancy leave shall provide her supervisor with her physician’s medical advice regarding leave commencement. She shall also visit a designated health care provider for further medical consultation if requested to do so by her supervisor.

Provide the TB Unit with guidance on use of medications when the U.S. Food and Drug Administration (FDA) or Centers for Disease Control and Prevention (CDC) issues reports of medications with concerning trace elements.
2. Offer technical assistance to other healthcare providers statewide, including community or private physicians, hospitals, and congregate settings. Technical assistance shall include providing guidance to clinicians that prescribe treatment regimens that do not align with DSHS treatment recommendations, and interpreting patient chest radiographs.
3. For consultations not addressed by the Mayo Clinic, provide written consultation to physicians in local and regional health departments who are managing persons with known or suspected TB and both their contacts. Consultation shall be provided to the requesting providers in accordance with the most current version of the Texas Tuberculosis (TB) Work Plan, the DSHS Standing Delegation Orders, TB Standards, TB Recommendations and TB Administration Resources located at: ▇▇▇▇://▇▇▇.▇▇▇▇.▇▇▇▇▇.▇▇▇/idcu/disease/tb/policies/.
4. Develop a process that identifies physicians with expertise in TB who are able to provide technical assistance and first-line consultation to local health departments and will increase capacity across the State of Texas. Document the process to ensure transparency in selection of eligible physicians and include an outline of competencies required of a TB medical consultant. Submit the written process to DSHS for review and approval.
Medical Consultation. The parties note that medical consultation, etc. may be arranged by the institutions in accordance with a decision by the institution concerned.
