Common use of Maternity Disability Leave Clause in Contracts

Maternity Disability Leave. 14.1.13.1 This leave commences with the onset of disablement due to pregnancy. The employee may claim sick leave pay and/or extended disability pay for no more than that limited period of time when the employee’s physician certified in writing on the form provided by the District that she was actually physically disabled from performing her duties because of pregnancy, miscarriage, childbirth, or recovery there from.

Maternity Disability Leave. 14.1.13.1 14.3.1 This leave commences with the onset of disablement disability due to pregnancy. The employee may claim sick leave pay and/or extended disability pay and differential sick leave compensation for no more than that limited period of time when the employee’s 's physician certified certifies in writing writing, on the form provided by the District County Superintendent, that she was actually physically disabled from performing her duties because of pregnancy, miscarriage, childbirth, or recovery there fromtherefrom or for those periods provided in subparagraph 14.1 or 14.2 above, whichever is shorter.

The employee may claim sick leave pay and/or extended disability differential pay for no more than that limited period of time when the employee’s 's physician certified certifies, in writing writing, on the form provided prescribed by the District District, that she was actually physically disabled from performing her duties because of pregnancy, miscarriage, abortion, childbirth, or and recovery there fromtherefrom.

The employee may claim sick leave pay and/or and extended disability pay for no more than that limited period of time when the employee’s 's physician certified or licensed mid-wife certifies, in writing writing, on the form provided by the District County Superintendent, that she was actually physically disabled from performing her duties because of pregnancy, miscarriage, childbirth, or recovery there from. In any event, this leave will not exceed those periods provided by paragraphs 10.1 or 10.2, above.
