MANUFACTURER’S CERTIFICATION. In addition to the Contractor’s Certification, a Manufacturer’s Certification must be completed and signed by the manufacturer before payment will be made to the Contractor for the delivered items. A Manufacturer’s Certification form identical to the form shown below must be used. The Contractor is not required to submit the completed and signed Manufacturer Certification form with their proposal. The Commonwealth shall have not obligation to pay for the item(s) until a properly completed and signed manufacturer’s certification is submitted for the delivered item.
MANUFACTURER’S CERTIFICATION. The City reserves the right to request from bidders/proposers a separate Manufacturer’s Certification of all statements made in the bid/proposal. Failure to provide such certification may result in the rejection of bid/proposal or termination of contract/agreement, for which the bidder/proposer must bear full liability.

MANUFACTURER’S CERTIFICATION. Respondent shall submit one (1) completed Manufacturer’s Certification form (Section 7.3), certifying that the Respondent is the Manufacturer for each Manufacturer / Brand Name they offer Commodities for as part of their Response. The Manufacturer’s Certification form must be executed by the Manufacturer only and may not be completed by the Dealer / Certified Representative. The Manufacturer’s Certification form shall be mailed to the Department per Section 3.8.1

MANUFACTURER’S CERTIFICATION. 7.1. The Contractor shall submit proof of Manufacturer certification in response to the qualifications questions located in Section 3 Attachment 2-A, stating that the Contractor is the Manufacturer or a Certified Representative of the Manufacturer, for each Manufacturer they represent under a resultant contract. The Manufacturer’s Certification must be executed by the Manufacturer(s) only, and may not be completed by the Contractor. Dealer agreements shall not be accepted in lieu of a Manufacturer’s Certification.
