Life and Accidental Death and Dismemberment. The Board shall purchase through a carrier licensed by the State of Ohio, a $20,000 policy. The Board thereof shall pay the full cost of this coverage and increases. Every employee shall have the opportunity to purchase additional Life and Accident Death and Dismemberment coverage and other insurance coverage established by the carrier, within coverage and cost limits set by the carrier, through payroll deduction. Employees must enroll for additional coverage in writing, to the Treasurer during the established open enrollment period for additional coverage to be provided in the next contract year.

Life and Accidental Death and Dismemberment. The City shall provide an amount of life and accidental death and dismemberment insurance equivalent to $20K Life to permanent, full-time employees. The City will make available to members the opportunity to purchase, at their own cost, additional term life insurance at group rates.
Life and Accidental Death and Dismemberment. (AD&D) Insurance. CMU Choices provides coverage options of 1, 1.5, 2, 3, and 4 times the bargaining unit member’s base salary, according to the terms of the policy. A bargaining unit member must elect a coverage level of at least 1 times the member’s base salary. CMU will contribute an amount per month per $1000 of coverage equivalent to 1.5 times the bargaining unit member’s base salary that will fully pay the premium for this amount of coverage. Each coverage option contains an equal amount of additional benefit in the form of AD&D coverage.
Life and Accidental Death and Dismemberment. (AD&D) Insurance Each bargaining unit member shall receive College-paid term life and AD&D insurance payable to his or her designated beneficiary(ies) in the amount equal to the employee’s annual salary. Additional term life and AD&D insurance up to three times the employee’s annual salary may be purchased at a fair market rate.
Life and Accidental Death and Dismemberment. The Board shall pay the cost of $10,000 worth of life insurance and an ADD policy for all employees that are employed thirty or more hours per week.
Life and Accidental Death and Dismemberment. (AD&D) Reductions - Benefits reduce eight (8%) per year for principals actively at work, beginning at age sixty-five (65) and will reduce an additional 8% each year thereafter subject to a minimum benefit of $2000. Amounts of the insurance in excess of $200,000 are subject to evidence of insurability satisfactory to Underwriter.

The Board shall provide all employees with ten thousand dollars (10,000.00) life insurance, which shall include a double indemnity clause for accidental death and dismemberment. In addition, the Board shall make available an unlimited amount at the employee’s own expense. (Individual rates may vary depending on the employee’s age).
Life and Accidental Death and Dismemberment. (AD&D) The Company will provide $30,000 in Group Life and AD&D benefits for each employee, effective the first day of the month following the date of hire.
Life and Accidental Death and Dismemberment. The Employer shall maintain life insurance and accidental death and dismemberment policy coverage comparable to the policy in effect for employees at the time of ratification of this Agreement, subject to insurance company limitations. If the Employer determines that it is appropriate to change carriers or policies, it shall consult with the Union before undertaking any such changes.
