Letter of Authorization. Each Party is responsible for obtaining a Letter of Authorization (LOA) from each End User initiating transfer of service from one Party to the other Party in accordance with applicable law. The Party obtaining the LOA from the End User will furnish it to the other Party upon request. The Party obtaining the LOA is required to maintain the original document, for a minimum of twenty-four (24) months from the date of signature. If there is a conflict between an End User and Carrier regarding the disconnection or provision of services, Frontier will honor the latest dated Letter of Authorization. If the End User’s service has not been disconnected and services have not yet been established, Carrier will be responsible to pay the applicable service order charge for any order it has placed. If the End User’s service has been disconnected and the End User’s service is to be restored with Frontier, Carrier will be responsible to pay the applicable nonrecurring charges as set forth in Frontier applicable tariff to restore the End User’s prior service with Frontier.
Letter of Authorization. 8.1 GTE will not release the Customer Service Record (CSR) containing Customer Priority Network Information (CPNI) to SETI on GTE end user customer accounts unless SETI first provides to GTE a written Letter of Authorization (LOA), signed by the end user customer, authorizing the release of such information to SETI or if state or federal law provides otherwise, in accordance with such law.

(“Momentum”) as agent for Momentum to act as its authorized agent for all matters pertaining to the number(s) populated in the appropriate fields. This agency includes disconnections of service and other requests as deemed necessary by Momentum to implement the services ordered from Momentum, including but not limited to: (1) securing information for activating, porting disconnecting, editing and transferring service for Customer, (2) securing information for the purposes of resolving technical issues for Customer, (3) securing information for activating, removing, changing and editing Customer’s directory listings.

(“Momentum”) as agent for Big Xxxxx Broadband to act as its authorized agent for all matters pertaining to the number(s) listed below. A LOA will be required before VERIZON will process an order for UNEs or Combinations provided in cases in which the subscriber currently receives local exchange or Exchange Access service from VERIZON or from a local service provider other than SPRINT. Such LOA may be a blanket LOA or such other form as agreed upon between VERIZON and SPRINT.

Letter of Authorization. 5.2.1. The Parties agree that it will not submit an order to move an End User Customer’s service from one Party to the other Party without the End User Customer’s permission, and that the requesting Party has verification from the End User Customer a Letter of Authorization (LOA), etc. that the End User Customer has agreed to the change in service.
