Common use of Insurance Plan Clause in Contracts

Insurance Plan. (a) A life insurance policy of $50,000 with a permanent total disability benefit provision, subject to the conditions and provisions of said policy, shall be provided for all Operators with 5 years or more of - service, the full premium cost of which shall be paid for by SFMTA. For Operators with 1 year or more but with less than 5 years of service a similar policy of $6,000 will be provided.
