Insurance on Employee. The Company shall be entitled to obtain and maintain, at the Company’s expense, key person life insurance on the life of the Employee, naming the Company as the beneficiary of such policy. Employee agrees to cooperate with the Company and take all reasonable actions necessary to obtain such insurance, such as taking usual and customary physical examinations and providing true and accurate personal, health related information for any application at no cost to Employee.

Insurance on Employee. A)The Executive agrees that the Company may continue or terminate and may periodically apply for and take out life, health, accident or other insurance on the Executive in its own name and at its own expense in whatever sum the Company may deem necessary to protect its interests. The Executive also agrees to submit to the customary medical examinations and to fill out, execute and deliver all applications or other instruments in writing as is reasonably required by any or all insurance companies to which any or all applications are made by or for the Company in order to aid the Company in procuring the insurance. The Executive further agrees that he shall have no right, title or interest in or to any of the insurance policies the Company may take out in his name.
Insurance on Employee. EMPLOYEE agrees that ANACOMP may, at its option and expense, obtain life insurance on the life of the EMPLOYEE and the ownership of all such policies and the proceeds therefrom shall be the sole property of ANACOMP. EMPLOYEE agrees to undergo a routine physical examination for insurance purposes within fifteen (15) days upon the request and at the expense of ANACOMP.

Such payment will be made by payroll deduction from pension checks, or by direct payment in the case of an early retiree.

  • Long Term Disability Insurance Plan The Employer shall provide a mutually acceptable long-term disability insurance plan, a copy of which shall appear in Appendix “A” – Long-Term Disability Insurance Plan.
