Home and Hospital Teachers. A. Home and Hospital teachers shall be compensated at the rate identified in Appendix 3.
Home and Hospital Teachers. ○ Home and Hospital Teachers will continue to provide support remotely or in person by mutual agreement between the practitioner and the family. ● Classroom Visitations: ○ Principals/administrators may attend virtual sessions by coordinating pre-arranged times with a practitioner. ■ In person classroom visits are allowable following health and safety guidelines. ○ No volunteers are allowed on campus. ○ No visitors are allowed on campus unless approved by the site administration. ● Site Access: ○ Classroom teachers will provide in-person instruction from their classrooms/school sites for the morning sessions and may work remotely the rest of the time. ○ Other certificated staff: If the practitioner prefers to work from the site, they may do so if the site can identify a safe space. ○ Social emotional staff: If the practitioner determines in person services are needed, the site administrator will ensure a confidential safe space is provided for the social. emotional staff member to provide therapy both in-person and virtually at the site. ● Grades and Marks: ○ Academic numeric grades and Successful Practices will be issued at each trimester and will be based on evidence from a combination of synchronous and asynchronous learning. ● Site Collaboration Time: ○ Utilizing the intent of Article 24 within the CBA, site leadership teams may utilize up to 75 minutes each Wednesday for collaboration and/or professional development. Site leadership teams should be coordinating with the staff to ensure that synchronous activities with students are not also occurring at this time. ● Preparation Time: ○ Classroom teacher will receive approximately*: ■ TK-5: 45 minutes M, T, Th, F and the remaining time on Wednesdays ■ Middle Years: 50 minutes M, T, Th, F and the remaining time on Wednesdays ■ Teachers will need to build the time into their schedules. Prep time will not be provided by an elementary specialist. ■ The remainder of the Wednesday time will be designated for prep time. Home and Hospital Teachers. B. Home and Hospital instruction is provided to students on an as-needed basis. Teachers of record shall be notified of students receiving Home and Hospital instruction and shall be given the first opportunity to take on the Home Hospital student. Compensation is set forth in Appendix B, Table IV. The bargaining unit member shall be indemnified and held harmless against any claims, causes of action, damages, grievances, administrative proceedings or any other litigation as provided by Division 3.6 (commencing with Section 810) of Title I of the Government Code.

  • Hospice g. Individuals whose permanent residence and principal work location are outside the State of Minnesota and outside of the service areas of the health plans participating in Advantage. If these individuals use the plan administrator’s national preferred provider organization in their area, services will be covered at Benefit Level Two. If a national preferred provider is not available in their area, services will be covered at Benefit Level Two through any other provider available in their area. If the national preferred provider organization is available but not used, benefits will be paid at the POS level described in paragraph “i” below.   • HEALTH CARE PLANS ‌ Notwithstanding the references to the Pacific Blue Cross Plans in this article, the parties agree that Employers, who are not currently providing benefits under the Pacific Blue Cross Plans may continue to provide the benefits through another carrier providing that the overall level of benefits is comparable to the level of benefits under the Pacific Blue Cross Plans.

  • Extended Health Care Plan (a) The Employer shall pay the monthly premium for regular employees entitled to coverage under a mutually acceptable Extended Health Care Plan.

  • Hospital This plan covers behavioral health services if you are inpatient at a general or specialty hospital. See Inpatient Services in Section 3 for additional information. Residential Treatment Facility This plan covers services at behavioral health residential treatment facilities, which provide: • clinical treatment; • medication evaluation management; and • 24-hour on site availability of health professional staff, as required by licensing regulations. Intermediate Care Services This plan covers intermediate care services, which are facility-based programs that are: • more intensive than traditional outpatient services; • less intensive than 24-hour inpatient hospital or residential treatment facility services; and • used as a step down from a higher level of care; or • used a step-up from standard care level of care. Intermediate care services include the following: • Partial Hospital Program (PHP) – PHPs are structured and medically supervised day, evening, or nighttime treatment programs providing individualized treatment plans. A PHP typically runs for five hours a day, five days per week. • Intensive Outpatient Program (IOP) – An IOP provides substantial clinical support for patients who are either in transition from a higher level of care or at risk for admission to a higher level of care. An IOP typically runs for three hours per day, three days per week.

  • Home Health Care This plan covers the following home care services when provided by a certified home healthcare agency: • nursing services; • services of a home health aide; • visits from a social worker; • medical supplies; and • physical, occupational and speech therapy.

  • Behavioral Health Services Behavioral health services include the evaluation, management, and treatment for a mental health or substance use disorder condition. For the purpose of this plan, substance use disorder does not include addiction to or abuse of tobacco and/or caffeine. Mental health or substance use disorders are those that are listed in the most updated volume of either: • the Diagnostic and Statistical Manual of Mental Disorders (DSM) published by the American Psychiatric Association; or • the International Classification of Disease Manual (ICD) published by the World Health Organization. This plan provides parity in benefits for behavioral healthcare services. Please see Section 10 for additional information regarding behavioral healthcare parity.   • Health Services At the time of employment and subject to (b) above, full credit for registered professional nursing experience in a school program shall be given. Full credit for registered professional nursing experience may be given, subject to approval by the Human Resources Division. Non-degree nurses shall be placed on the BA Track of the Teachers Salary Schedule and shall be ineligible for movement to any other track.
